Book excerpt: Since the beginning of the century when HAMBURGER, SCHLOSS MANN, and MORO first described food allergy in infants being fed with cow's milk, this topic has been the subject of very con troversial discussions among pediatricians. The dispute is illus trated by markedly fluctuating incidence figures, ranging from denial of the disease to incidence rates up to one in ten infants. The explanation for the differing incidence figures lies in the lack of a single laboratory test which is simple as well as applicable and reliable for all clinical and immunological reactions. Even though the classic allergic hypersensitivity reactions mediated by specific IgE antibodies are relatively clearly defined, there still exist other more complex immune responses which are more dif ficult to recognize. In the fall of 1987 internationally renowned experts from var ious fields met to define and discuss the fundamentals, organic manifestations, and the current status of diagnosis, treatment, and prevention of food allergy in childhood. The results have been collected in the volume in hand, in hopes that it will encourage more public involvement in the discussion of this illness, which is fortunately mostly transient.

Original papers dealing with different aspects of the subject are appearing at an increasingly rapid rate in a very wide range of journals, and it becomes more and more difficult for the geneticist and virtually impossible for the non geneticist to keep track of the develop ments. Furthermore, new observations and discoveries relevant to an overall understanding of the subject result from investigations using very diverse techniques and methodologies and originating in a variety of different disciplines. Thus, investigations in such various fields as enzymology, immunology, protein chemistry, cytology, pediatrics, neurology, internal medicine, anthropology, and mathematical and statistical genetics, to name but a few, have each contributed results and ideas of general significance to the study of human genetics. Not surprisingly it is often difficult for workers in one branch of the subject to assess and assimilate findings made in another. This can be a serious limiting factor on the rate of progress. Thus, there appears to be a real need for critical review articles which summarize the positions reached in different areas, and it is hoped that "Advances in Human Genetics" will help to meet this requirement. Each of the contributors has been asked to write an account of the position that has been reached in the investigations of a specific topic in one of the branches of human genetics.

In the early stages of the research many investigators suspected the existence of a complex series of transplantation antigens, but it was widely believed that these antigens would not be well-defined even in this century. Yet in the last two decades as many as 124 different HLA antigens determined by at least 7 very closely linked genes located on the short arm of chromosome 6 have been identified and subsequently agreed upon by an international nomenclature committee. 1 Extensive international collaboration fueled by the potential clinical application of these antigens to clinical transplantation has advanced the field rapidly. There were nine inter national histocompatibility workshops held during this period. Although iden tification of HLA antigens was of primary clinical importance in transplantation 2 and of great basic interest in human genetics and anthropology, a rather un expected bonus has been the determination that HLA antigens are associated with disease susceptibility to a greater extent than any other known genetic marker in man. In the past, many genetic polymorphisms have been suspected to be associated with diseases. The most extensively studied markers are blood groups, enzymes, and serum proteins. A comprehensive account of published studies, totalling approximately 1,000, of these markers is available in a book by Mourant et al.
