Book excerpt: The lymphatic system serves as the primary route for the metastasis of breast cancer, and the extent of lymph node involvement is a key prognostic factor for the outcome of the disease. Whereas the significance of angiogenesis for tumor progression has been well documented, the ability of tumor cells to induce the growth of lymphatic vessels, (lymphangiogenesis) and the presence of intratumoral lymphatic vessels have been questioned. We demonstrate occurrence of intratumoral lymphangiogenesis within human breast cancers alter orthotopic transplantation onto nude mice, using a novel marker for lymphatic endothelium, LYVE-1-I. We show using green fluorescent protein-tagging that breast tumor cells which overexpress vascular endothelial growth factor-C (VEGF-C), display a potent increase in intratumoral lymphangiogenesis, and significantly enhanced metastasis to both regional lymph nodes and lungs. Furthermore we have found that the degree of tumor lymphangiogenesis is highly correlated with the extent of both lymph node and lung metastases. These results establish the occurrence and biological significance of intratumoral lymphangiogenesis in breast cancer and identify VEGF-C as a molecular link between tumor lymphangiogenesis and metastasis.

Book excerpt: Metastasis significantly contributes to cancer mortality and is facilitated by lymphangiogenesis. Recent reports indicate that VEGF-A can modulate this process. We therefore evaluated VEGF-A neutralization as a potential anti-lymphangiogenic treatment in an orthotopic model of human breast cancer MDA-MB-231. Anti-VEGF-A therapy caused an 80% reduction in lymphatic vessel density and inhibited metastasis. VEGF-A neutralization decreased expression of VEGF-C by 28% in cultured MDA-MB 231 cells but not affect global VEGF-C expression in vivo. Immunofluorescent staining of lymphatic vessels demonstrated robust expression of VEGFR-3 on LYVE-1-positive vessels in control tumors. However, decreased and aberrant expression of VEGFR-3 was detected on lymphatic vessels in VEGF-A depleted tumors. RT-PCR analysis revealed three-fold decreased expression of Ang-2 in treated tumors which correlated with decreased VEGFR-3 transcript. The study demonstrates VEGF-A neutralization is a potential target for anti-metastatic therapy and identifies a novel mechanism for VEGF-A dependent modulation of lymphangiogenesis.

In addition to its role in the regulation of vascular volume, transcapillary fluid filtration also allows for continuous turnover of water bathing tissue cells, providing the medium for diffusional flux of oxygen and nutrients required for cellular metabolism and removal of metabolic byproducts. Transendothelial volume flow has also been shown to influence vascular smooth muscle tone in arterioles, hydraulic conductivity in capillaries, and neutrophil transmigration across postcapillary venules, while the flow of this filtrate through the interstitial spaces functions to modify the activities of parenchymal, resident tissue, and metastasizing tumor cells. Likewise, the flow of lymph, which is driven by capillary filtration, is important for the transport of immune and tumor cells, antigen delivery to lymph nodes, and for return of filtered fluid and extravasated proteins to the blood. Given this background, the aims of this treatise are to summarize our current understanding of the factors involved in the regulation of transcapillary fluid movement, how fluid movements across the endothelial barrier and through the interstitium and lymphatic vessels influence cell function and behavior, and the pathophysiology of edema formation.
