Objectif : évaluer par une revue de la littérature la prise en charge par le médecin généraliste du surpoids et de l'obésité de l'enfant migrant ou issu de milieux défavorisés. Méthodes : l'équation de recherche a été déterminée en PubMed. Sur les 1953 études, les textes intégraux de 33 d'entre eux ont été récupérés. 5 articles ont été sélectionnés. Résultats : deux études sont anglo-saxonnes, deux sont australiennes et une est américaine. Si la majorité des médecins croient en leur rôle dans la gestion de l'obésité de l'enfant, beaucoup pensent que leur impact est limité. La plupart n'osent pas aborder le sujet en consultation. Très peu pèsent et mesurent les enfants systématiquement. Moins d'un tiers des praticiens connaissent les recommandations de leur pays sur la surcharge pondérale de l'enfant et moins d'un quart les appliquent. Les médecins manquent de temps et de formation. Les directives peuvent ne pas être comprises ou mal vécues par les familles issues de milieux précaires. Conclusion : il est nécessaire de développer des programmes de prévention ciblant les enfants issus de milieux socio-économiques défavorisés.

Une approche comparée de deux groupes d'enfants, de 6 ans 4 mois, à 11 ans 8 mois (23 enfants obèses et 23 enfants de poids normal) permet de mettre à l'épreuve différentes hypothèses concernant certaines caractéristiques psychologiques et les facteurs de risque qui leur sont associes... un dispositif méthodologique précis a été applique, constitue d'entretiens semi-directifs parent(s)enfant, d'un questionnaire de comportement alimentaire et d'épreuves projectives de personnalité. Des différences significatives entre obèses et normo pondéraux sont confirmées: elles mettent en lumière des particularités dans les conduites alimentaires présentées par les enfants obèses, mais soulignent notamment la précocité d'agencement de la conflictualité orale chez ces enfants. Ces résultats trouvent également tout leur intérêt dans les implications entrevues, tant sur le plan thérapeutique que préventif: s'il s'agit en effet de prendre en considération les habitudes alimentaires des enfants obèses, leur remaniement nécessite cependant une compréhension des mécanismes dynamiques intrapsychiques qui sous-tendent l'acte alimentaire.
