Book excerpt: Introduction : Chaque année, on estime à 15 millions le nombre d'enfants naissant prématurément dans le monde, la prématurité restant d'une part la principale cause de mortalité chez l'enfant de moins de 5 ans et d'autre part, ses conséquences pouvant être très importantes sur la vie future de l'enfant. Objectif : Le but de cette étude est de comparer l'efficacité de différents types de stimulations sensorimotrices associées ou non à des stimulations tactiles (massage) dont l'effet positif a déjà été prouvé sur le développement du nouveau-né prématuré Méthodologie de recherche : Les articles ont été explorés principalement sur PubMEd, Cochrane et PEDro. Seuls les essais contrôlés randomisés faisant intervenir des stimulations sensorimotrices dans la prise en charge de l'enfant prématuré avec un niveau de preuve supérieur ou égal à 5/10 sur l'échelle de PEDro et ayant été publiés à partir de 2009 ont été sélectionnés. Le critère principal est la prise de poids tandis que les critères secondaires sont la durée d'hospitalisation et la fonction motrice. Résultats et analyse : L'association de stimulations tactiles et sensorimotrices réalisés respectivement dans cet ordre pendant une durée de 15 min, deux fois par jour permettrait d'obtenir des résultats optimaux sur le gain de poids, la diminution de la durée d'hospitalisation et l'amélioration des fonctions motrices. Discussion : La faible taille des échantillons de ces études et la faible quantité de données scientifiques disponibles à ce sujet ne permettent pas d'avoir une vision représentative de la population étudiée. Cependant les résultats obtenus dans cette revue sont encourageants et indiquent qu'il serait intéressant d'approfondir les recherches sur ce sujet avec un nombre de sujets inclus plus important.

Book excerpt: Cette étude longitudinale évalue chez une population d'enfants nés prématurément l'effet d'une intervention précoce sur les interactions parent-enfant et le développement social de l'enfant. Cette intervention aide le parent à observer lui-même ses forces interactives et les réactions de son enfant à l'aide de mises en situation filmées et de rétroaction vidéo. L'échantillon est constitué de 72 familles évaluées à 8 (pré-test), 11 (post-test) et 18 (suivi) mois (âge corrigé), dont 54 enfants sont suivis à la maternelle. La sensibilité parentale est évaluée à 8, 11 et 18 mois à l'aide du Q-sort de sensibilité parentale et les interactions parent-enfant sont filmées lors d'une rencontre à domicile et codifiées en laboratoire. L'attachement est évalué à 18 mois à l'aide du Q-sort des comportements de l'enfant. Les problèmes au plan social sont évalués à la maternelle à l'aide du Child Behavior Checklist complété par la mère et le père, ainsi que par le Teacher's Report Form et le Profil socio-affectif, complété par l'enseignant. Suite à l'intervention, la sensibilité parentale est supérieure chez les parents du groupe intervention que chez ceux du groupe contrôle. Celle-ci semble s'améliorer chez le groupe intervention alors qu'elle semble se détériorer chez le groupe contrôle. De plus, les parents du groupe intervention ont des comportements plus positifs que ceux du groupe contrôle. À 18 mois, l'attachement de l'enfant semble amélioré, ainsi que les comportements de maintien et de recherche de contacts, qui constituent un indicateur de la qualité des interactions parent-enfant. Bien qu'aucun effet direct de l'intervention sur les problèmes de comportements à la maternelle soit observé, les problèmes au plan social de l'enfant à la maternelle sont liés à la sécurité d'attachement de l'enfant et cette dernière variable constitue une variable médiatrice de la relation entre la sensibilité parentale et les problèmes internalisés, ce qui semble démontrer que l'intervention CoNaître a le potentiel de réduire les problèmes internalisés chez les enfants nés prématurément à la maternelle. Les résultats soutiennent donc que l'intervention proposée est efficace pour amener le parent à avoir des interactions plus optimales avec son enfant en prévenant une détérioration, et même, en améliorant la sensibilité des parents, entraînant une amélioration du développement social des enfants nés prématurément.
