Cette étude comparative de la thèse de Marion Dubruel réalisée en 2007 sur l'installation des médecins généralistes libéraux en Gironde en 2003 - 2004 et 2005, tente de voir si depuis 6 ans le choix de l'installation et du lieu d'installation ont évolué. A travers un questionnaire détaillé, j'essaie de répondre à plusieurs questions : Qui s'installent à l'heure actuelle, pourquoi et où ?, Dans quelles conditions s'installent-ils ?, Avec l'aide de qui ou de quoi s'installent-ils ?. Dans ma première partie sur la démographie médicale, deux points se dégagent nettement : - une féminisation importante du métier qui va s'accentuer dans les années à venir, - des départs accrus à la retraite depuis 10 ans qui vont se poursuivre, avec un choix du cumul activité libérale et retraite pour de nombreux praticiens. De plus, si on s'intéresse à l'âge de l'installation des médecins généralistes en libéral, celui-ci ne cesse d'augmenter. Ce travail essaie de relater les principales motivations et principaux freins à une installation rapide en milieu libéral, parmi lesquels le problème de la permanence des soins . De nos jours, les pouvoirs publics concentrent leurs efforts sur le fait de maintenir une offre de soins suffisante dans les territoires fragiles, en développant entre autres les maisons de santé pluridisciplinaires. Il est primordial de s'intéresser à ces zones désertifiées, mais il est nécessaire également de maintenir un nombre suffisant d'installations. Pour aider les médecins à s'installer, il est important de trouver une solution pour accompagner ces jeunes après leurs études, dans leurs premières années que ce soit pour trouver une offre d'association ou les aider dans les questions administratives, financières et juridiques. En effet, après une dizaine d'années passées au sein d'une structure telle q'un hôpital, on demande à de jeunes médecins de s'engager dans une entreprise libérale, sans avoir eu aucun enseignement ou presque, concernant la gestion administrative et financière d'un cabinet, ou le coût de rachat d'une patientèle par exemple. Les résultats de cette thèse montrent bien que ces jeunes médecins sont demandeurs d'aides spécialisées de bonne qualité. En outre, par rapport à la thèse de Marion Dubruel, j'aborde des questions d'actualité comme la collaboration, le salariat en essayant de mettre en exergue les différents points positifs et négatifs de ces types de fonctionnement. Enfin, toutes les études faites sur le sujet constatent que le choix de l'installation rend les médecins heureux.
