The Inventory for Assessing the Process of Cultural Competence among Healthcare Professionals - Revised (IAPCC-R) was used to collect self-reported data from participants attending a two-year ADN nursing program in southwestern North Carolina. IAPCC-R scores were significantly higher for sophomores than for freshmen. Analysis determined grade level as the primary factor influencing scores. This study helps expand the use of the IAPCC-R to the ADN population. [author abstract slightly modified].

Book excerpt: The purpose of this study was to determine whether a course in cultural competency would impact associate degree nursing students' perceptions of their abilities to provide care to culturally diverse clients. Ethnographic interviews with nursing faculty, graduates and students were utilized in course development. The cultural competency course was evaluated through the use of a pretest-posttest style survey. The interviews with faculty revealed themes that influenced the cultural competency course: critical reflection, communication, local cultural and ethnic groups, community resources, and mentoring. Most graduates and nursing students demonstrated a need for cultural competency, however those with the most cross-cultural experience provided the most culturally sensitive descriptions of their nursing experiences. The survey results confirmed findings from participant observation that the course positively impacted nursing students' comfort level, knowledge, and awareness of issues surrounding cultural competency. Based on the results of this study, recommendations have been made to continue cultural competency education in associate degree nursing and to add local cross-cultural experiences as a means of applying what is learned in the classroom.

Campinha-Bacote's (2003) Process of Cultural Competence in the Delivery of Heathcare Services Model provided the organizing framework for the study. A revised version of Sealey's (2003) Cultural Diversity Questionnaire for Nurse Educators was used for the study. Out of an accessible population of 503 nurse educators teaching in associate degree nursing programs in four types of educational institutions in Ohio, 137 responded to the questionnaire. The forty-one item Likert type questionnaire along with eleven questions regarding demographic and professional characteristics was administered via the Internet over a three week period. The findings from this study indicated that associate degree nurse educators in Ohio "agree" that they possess cultural awareness, cultural knowledge, cultural skill, and culture desire and are undecided regarding their level of engagement in cultural encounters. The findings also indicated that the participants "agree" that their teaching behaviors reflect cultural competence. The study results provide direction for professional development programs for nurse educators in the area of cultural diversity and support for hiring and retaining a culturally diverse nursing faculty."--Abstract.

An analysis of one university's school of nursing program curriculum was conducted to measure the cultural awareness and competency of nursing students progressing through BSN and ADN accredited programs, using the Cultural Awareness Scale (CAS) survey and a qualitative survey. Results indicated BSN students and senior level students score higher on average compared to ADN students and junior level students respectively. However, low means overall and only 15% to 18% of participants stating they felt very prepared to provide culturally competent care suggested a lack of effectiveness and need for further investigation by the school of nursing to make necessary improvements in their cultural awareness and competency curriculum.

The literature showed a lack of empirical studies investigating the confidence of nurses and nursing students in providing culturally competent care. Therefore, the research question was, "What are Southwest Missouri senior baccalaureate student nurses' self-perceptions of their competence in providing cultural care for diverse ethnic clients?" A convenience sample of baccalaureate nursing students, enrolled during the fall 2009 semester, from two universities in Southwestern Missouri were recruited for this study. Students completed the Bernal and Froman's Cultural Self-efficacy Scale (CSES). This study found that the participants' self-perceived cultural knowledge, concepts, patterns, and transcultural nursing skills ranged between 3 (neutral) and 4 (moderate confidence) on the Likert scale 1 to 5. Based on the computed subscales of the CSES, the score of the total self-efficacy was 194.53, which means the participants rated themselves as neutral to moderately competent. The study found that there is a minimal increase in the self-perceived confidence levels of cultural competence compared to other studies in the Midwest.

Book excerpt: It has been decades since the concept of cultural competency was introduced in the literature, yet nursing faculty still show low levels of cultural competence. The purpose of this study was to assess the level of cultural competence among nursing faculty teaching in collegiate schools of nursing in the U.S. and to identify and examine demographic factors that are associated with the cultural competence level of nursing faculty. The study design was a descriptive, correlational, non-experimental survey design with a sample of nursing faculty (n=461) teaching in Commission on Collegiate Nursing Education (CCNE) accredited baccalaureate nursing programs in the U.S. Data were collected using "Cultural Diversity Questionnaire for Nurse Educators Revised" CDQNE-R. The guided theoretical framework of this study was Campinha-Bacote's (2010) Process of Cultural Competence in the Delivery of Healthcare Services Model. Statistical analysis was done with SAS 9.3© (SAS Institute Inc., Cary, NC). The overall cultural competence (OCC) mean score for the study sample =166.3 ± 19.5 indicating that nursing faculty who teach at BSN programs across the U.S. have a moderate level of cultural competence. The highest indexes were for the Cultural Knowledge Subscale M= 43.53 ± 6.2, and the Cultural Teaching Behaviors Subscale M= 42.10 ± 4.98. The low indexes were for the Cultural Skills Subscale M= 31.53 ± 4.41, and the Cultural Encounter Subscale M =22.50 ± 4.42. Regression analysis found that selected demographic variables were predictive of OCC mean score. These variables included residing in a different country, language other than English, nursing specialty, highest degree attained, cultural continuing education, including cultural content in current nursing program, level of incorporating cultural content in current program, and assessing student needs and were significantly predictive of OCC mean score after adjusting for race, age group, and gender. The study also found that the transcultural teaching behaviors subscale was significantly associated with the OCC scale, adjusting for all other variables. The explained variance for the models ranged from 39% to 86%. Future studies are needed to identify factors that contribute to cultural competence from faculty and student perspectives; to identify the best-standardized evaluation tool to assess students' cultural competence levels; and to identify the best educational strategies to teach cultural competency in nursing education programs.

The 3-D Model for Culturally Congruent Care was the framework used for this quantitative descriptive correlational study. The premise of the model is to draw upon the knowledge that culturally competent care derives from the healthcare provider and client level, an essential factor to the delivery of culturally competent healthcare. A convenience sample of 74 student nurses from two separate cohorts enrolled in a diploma-based program were asked to complete the Cultural Competence Assessment (CCA) survey as a pre-intervention (2017 Cohort) and post-intervention (2016 Cohort).Data from the CCA were analyzed utilizing measures of central tendency and bivariate statistics. This analysis revealed significant associations between both groups of students and levels of cultural awareness and sensitivity. The results of this study affirm the value of both the classroom and clinical immersion experience in the development of cultural competence among student nurses.
