Book excerpt: Implementing safety practices in healthcare saves lives and improves the quality of care: it is therefore vital to apply good clinical practices, such as the WHO surgical checklist, to adopt the most appropriate measures for the prevention of assistance-related risks, and to identify the potential ones using tools such as reporting & learning systems. The culture of safety in the care environment and of human factors influencing it should be developed from the beginning of medical studies and in the first years of professional practice, in order to have the maximum impact on clinicians' and nurses' behavior. Medical errors tend to vary with the level of proficiency and experience, and this must be taken into account in adverse events prevention. Human factors assume a decisive importance in resilient organizations, and an understanding of risk control and containment is fundamental for all medical and surgical specialties. This open access book offers recommendations and examples of how to improve patient safety by changing practices, introducing organizational and technological innovations, and creating effective, patient-centered, timely, efficient, and equitable care systems, in order to spread the quality and patient safety culture among the new generation of healthcare professionals, and is intended for residents and young professionals in different clinical specialties.

This book was released on 2015-12-29 with total page 473 pages. Available in PDF, EPUB and Kindle. Book excerpt: Getting the right diagnosis is a key aspect of health care - it provides an explanation of a patient's health problem and informs subsequent health care decisions. The diagnostic process is a complex, collaborative activity that involves clinical reasoning and information gathering to determine a patient's health problem. According to Improving Diagnosis in Health Care, diagnostic errors-inaccurate or delayed diagnoses-persist throughout all settings of care and continue to harm an unacceptable number of patients. It is likely that most people will experience at least one diagnostic error in their lifetime, sometimes with devastating consequences. Diagnostic errors may cause harm to patients by preventing or delaying appropriate treatment, providing unnecessary or harmful treatment, or resulting in psychological or financial repercussions. The committee concluded that improving the diagnostic process is not only possible, but also represents a moral, professional, and public health imperative. Improving Diagnosis in Health Care, a continuation of the landmark Institute of Medicine reports To Err Is Human (2000) and Crossing the Quality Chasm (2001), finds that diagnosis-and, in particular, the occurrence of diagnostic errorsâ€"has been largely unappreciated in efforts to improve the quality and safety of health care. Without a dedicated focus on improving diagnosis, diagnostic errors will likely worsen as the delivery of health care and the diagnostic process continue to increase in complexity. Just as the diagnostic process is a collaborative activity, improving diagnosis will require collaboration and a widespread commitment to change among health care professionals, health care organizations, patients and their families, researchers, and policy makers. The recommendations of Improving Diagnosis in Health Care contribute to the growing momentum for change in this crucial area of health care quality and safety.

Book excerpt: Excerpt from Clinical Cases: Medical and Surgical Throughout his professional life Dr. Stone kept a careful record of all cases occurring in his own practice, or those he saw in consultation. During the last year of his life he wrote out such of these cases as seemed to him of interest, and worthy of record, intending to publish them together with some contributions he had made to The New York Medical Journal. After his death the manuscript was found almost ready to go into the printer's hand. This volume is now issued at the urgent request of some of his medical friends in the hope that its perusal may not prove entirely devoid of interest to the profession at large. Had the author been permitted to supervise the issue of this volume no doubt many changes would have suggested themselves to him, as it was his intention to have given the cases a careful revision. But few alterations have been made in the manuscript, only a word here and there, or a sentence has been changed - it having been thought best to leave the cases as narrated by him. With these few remarks the volume is submitted to the profession. About the Publisher Forgotten Books publishes hundreds of thousands of rare and classic books. Book excerpt: A guide to "when to" and "what to" rather than "how to," this book provides evidence-based surgical reviews to provide credible answers to age-old surgical management questions. The management issues presented are oriented toward interventions and use evidence-based techniques to assess the safety and efficacy of new treatments and rehabilitative or preventative interventions. Each chapter is organized around the key questions essential to delineating the current status of evidence related to the subject reviewed. Publications from the past decade are cited that provide Level I and II evidence using the Oxford scale. Throughout Elective General Surgery, careful assessment of the validity of intervention studies and the strength of the evidence that they provide underlies the choices of cited publications. The information presented in this volume guides the scientific surgeon in providing state-of-the-art care and in optimizing the use of medical resources without losing sight of the need to address the unique needs of individual patients.

Book excerpt: Evidence base in 2015 remains a subject of controversy for surgeons related to its application in surgery that cannot be approached as it is in medical evidence. Academic surgeons acknowledge that evidence base is necessary and private practitioners know it is woven into the fabric of their practice. Dr. Lisa Ishii and Dr. Travis Tollefson, editors of this publication, are at the forefront of clinical use of and research into evidence based surgery. The Oxford Centre system of evidence is used for this issue. Currently, evidence is dominant in the reconstructive aspect, moreso than the cosmetic aspect; as such, this resource focuses on the nerve and microvascular procedures. Topics include Facial vascular anomalies, Cleft lip and palate; Trauma; Facial reanimation; System reviews and metanalyses; and Skin care, Laser treatments; and Rhinoplasty. Audience for this resource is facial plastic surgeons, otolaryngologists, plastic surgeons, laser therapists, dermatologists, and skin researchers.
