Book excerpt: Trata-se de um estudo qualitativo, na perspectiva do materialismo histórico e dialético, com objetivo de analisar os elementos qualitativos do processo de trabalho que são imprescindíveis na definição dos parâmetros de dimensionamento da força de trabalho de enfermagem, em unidade de internação clínico-cirúrgica. O estudo justifica-se porque um dos grandes problemas enfrentados pela gerência de enfermagem, no cotidiano dos serviços de saúde, é a definição qualitativa e quantitativa da força de trabalho de enfermagem necessária para garantir uma assistência de qualidade, que leve em conta as necessidades dos trabalhadores e usuários. Além disso, os estudos disponíveis na literatura apontam, predominantemente, parâmetros quantitativos, os quais têm sido insuficientes para enfrentar os desafios presentes na prática assistencial. A investigação foi desenvolvida em um hospital privado da região sudeste do Brasil, a partir da visão de diferentes atores sociais: sujeitos-do-cuidado/usuários, trabalhadores de enfermagem, gerente de enfermagem, gerentes da instituição e outros profissionais de saúde que interagem diretamente com a enfermagem nas unidades. Os dados foram coletados em duas unidades de internação clínico-cirúrgica, de adultos, no ano 2006, utilizando, como instrumentos, a análise documental, a observação sistemática e entrevistas semi-estruturadas com 22 sujeitos. Sustenta-se na teorização sobre o processo de trabalho em enfermagem e em saúde, e no debate nacional e internacional, sobre dimensionamento da força de trabalho de enfermagem. A pesquisa respeitou todos os preceitos éticos, nos termos da Resolução nð 196 do Conselho Nacional de Saúde, e o relatório final é fiel à realidade e experiências vivenciadas, mas preserva o anonimato das(os) informantes e da instituição estudada. Para a sistematização dos dados foi realizada a análise temática do conteúdo das entrevistas, identificando as categorias e subcategorias que expressassem conteúdos significativos para o objetivo da pesquisa. Os resultados evidenciaram que a questão do dimensionamento da força de trabalho em enfermagem é central, para os administradores, para os trabalhadores e, também, para os sujeitos do cuidado, ou seja, os usuários dos serviços. Evidenciou igualmente que a dimensão qualitativa é imprescindível na definição de parâmetros de dimensionamento da força de trabalho em enfermagem, especialmente pela característica do trabalho em saúde, que trata da vida humana, que envolve imprevisibilidade e relação entre seres humanos que são distintos # subjetiva, social e culturalmente. Foram identificadas seis dimensões a serem consideradas na definição de parâmetros para o cálculo da força de trabalho em enfermagem. A primeira dimensão é a econômica, que é amplamente reconhecida por estudiosos de vários matizes teóricos, incluindo a análise da dinâmica do setor de serviços e as distinções entre o setor público e o setor privado. A segunda dimensão diz respeito aos sujeitos-trabalhadores. Inclui as condições de trabalho como salário, jornada e turno de trabalho, recursos materiais, equipamentos e condições ambientais; as condições de saúde dos trabalhadores; e o uso que esses sujeitos fazem de suas capacidades e das oportunidades de desenvolvimento permanente. Uma terceira dimensão refere-se aos cenários de trabalho que podem demandar maior exploração e desgaste, ou serem mais adequados, permitindo maior aproximação com a dimensão criativa e satisfatória do trabalho. A quarta dimensão diz respeito à qualidade do produto do trabalho, ou seja, aos seus resultados, e à percepção de quem recebe o cuidado. A quinta dimensão diz respeito aos instrumentos e dinâmicas de organização do trabalho, levando em conta o trabalho prescrito e o trabalho real. Deve-se considerar, nessa dimensão, o saber tecnológico disponível, as relações de trabalho e o processo de comunicação. E a sexta dimensão é aquela que envolve o objeto de trabalho da enfermagem, diz respeito à visão de mundo e necessidades do sujeito-do-cuidado. Os resultados da pesquisa demonstraram que, apesar da importância das fórmulas matemáticas para o dimensionamento da força de trabalho em enfermagem, as mesmas são insuficientes para a gestão cotidiana do trabalho. A sua aplicação, sem incluir as seis dimensões apontadas, distancia-se da possibilidade de aproximação com um trabalho livre de risco para ostrabalhadores e para os sujeitos-do- cuidado. This qualitative study is written from the perspective of historical and dialectic materialism. Its objective is to analyze the essential qualitative elements of the work process in the definition of the parameters of dimensioning the nursing workforce in a clinical-surgical overnight ward. The study is justified because it deals with one of the major problems faced by nursing management in day-to-day health care services; the qualitative and quantitative definition of the nursing workforce necessary to guarantee quality care, which in turn takes into consideration the needs of health care workers and users. Beyond this, the available studies in literature predominately point out quantitative parameters, which have been insufficient in facing the challenges present in care practice. The investigation was developed in a private-sector hospital in the southeast region of Brazil, based on the perspective of different social agents: subjects-of-care/patients, nursing workers, nursing manager, institutional managers, and other health care professionals who interact directly with nurses of these units. The data was collected in two clinical-surgical overnight wards for adults in 2006, utilizing document analysis, systematic observation, and semi-structured interviews with 22 subjects. The research was carried out respecting all ethical precepts in the terms of Resolution 196 of the Brazilian National Health Care Council, and the final report is true tothe reality and experiences lived by its participants while preserving the anonymity of the informants and the studied institution. The data were analyzed by the thematic analysis of the interview content, identifying the categories and sub-categories that express significant content concerning the objective of the study. The results point out that the question of the dimension of the nursing workforce is central to administrators, workers, and also to patients, who are the users of these services. It also evidenced that the qualitative dimension is essential in defining the parameters of dimensioning the nursing workforce, especially concerning the characteristic of health care work that deals with human life, which involves unpredictability and relationship between human beings who are unique in subjective, social, and cultural terms. The results of this study suggest that six dimensions should be considered in the definition of parameters for nursing workforce calculation. The first dimension is economic, which is amply recognized by scholars of various theoretical matrixes, including the analysis of the dynamic within the service sector and the distinctions among the public and private sectors. The second dimension deals with the subjects/workers. It includes the work conditions, such as salary, shifts, materials, equipment, and environmental conditions; the health conditions of the workers; and the use that these subjects make of their capacities and the opportunities for permanent development. A third dimension refers to the work scenarios that may demand greater exploration and wear, or that are more adequate in order to permit greater approximation with the creative and satisfactory dimension of work. The fourth dimension deals with the quality of the product of work, or rather its results and the perception of whomever receives the care given. The fifth dimension concerns the instruments and dynamics of the organization of the work, taking into considerationprescribed work and real work. One must consider in this dimension the available technological knowledge, the work relationships, and the communication process. The sixth dimension is that which involves the object of nursing work, dealing with the view of the world and the needs of the subject-of-care/patients. The results of this study demonstrate that although mathematical formulas are important for dimensioning the nursing workforce, they are not sufficient in order to execute day-to-day management of nursing work. Their application, without including the six dimensions pointed out in this study, distances nursing from the possibility of approximation with work that is free from risk to its workers and topatients.

Contudo, continuam a observar-se diferenças entre grupos étnicos no que diz respeito a padrões de saúde e condições de acesso aos serviços disponíveis. Um relatório recente da Organização Mundial para as Migrações (IOM, 2005) alerta para a necessidade de se adoptar na Europa políticas de saúde integradoras e “culturalmente sensíveis”. Isto é, políticas que visem o desenvolvimento e prestação de serviços de saúde sensíveis ao background social, cultural e religioso dos utentes. Como é defendido no mesmo relatório, este tipo de abordagem assume-se fundamental numa Europa cada vez mais multicultural e mais dependente de uma força de trabalho estrangeira activa e saudável. Em linhas gerais, têm vindo a ser defendidas dois tipos de abordagens no que respeita à promoção de políticas e do acesso dos imigrantes aos serviços de saúde. A primeira tem vindo a assumir uma ideologia “assimilacionista”, ou seja, é defendida a adaptação dos imigrantes aos cuidados de saúde existentes na sociedade de acolhimento. Já a segunda abordagem assume uma posição “multiculturalista” que defende a implementação de serviços adaptados às necessidades dos utentes de acordo com uma perspectiva sensível às diferenças culturais. Um exemplo deste segundo tipo de abordagem é o caso da Holanda, onde foram criados serviços específicos para imigrantes, entre eles, um centro para a promoção de conhecimento sobre a saúde imigrante com “mediadores culturais”. Neste âmbito, este trabalho procurou, a partir de estudos de caso de imigrantes cabo-verdianos, analisar os serviços de saúde da Holanda. Para este efeito, foram analisadas as perspectivas dos cabo-verdianos relativamente às características que definem um bom médico, assim como as suas expectativas e experiências com profissionais de saúde na Holanda. Ainda que o caso holandês seja um exemplo de uma abordagem “multiculturalista”, no trabalho de terreno identificaram-se três barreiras no acesso de imigrantes aos cuidados de saúde: a falta de informação sobre os serviços de saúde disponíveis; problemas de relação entre utentes de origem imigrante e os profissionais de saúde; e, finalmente, dificuldades sentidas pelos os utentes imigrantes em lidar com a condicionalidade da ajuda prestada. Estes problemas encontram-se profundamente interligados e estão relacionados com dois tipos de factores. Ao nível macro, estes incluem características organizacionais e estruturais do sistema de saúde holandês. Ao nível micro, os problemas encontrados derivam do desfasamento de expectativas entre caboverdianos e profissionais de saúde relativamente à forma pela qual os cuidados de saúde são prestados. Apesar das barreiras encontradas, os cabo-verdianos não poupam esforços para beneficiar de boa saúde, colocando em prática duas estratégias com o objectivo de aceder a cuidados de saúde de boa qualidade. Por um lado, os cabo-verdianos tentam adaptar o seu comportamento enquanto utentes dos serviços (help-seeking behaviour) ao modelo prevalecente na Holanda. Por outro lado, recorrem ao uso de cuidados de saúde transnacionais noutros países da Europa, incluindo Portugal e França. Estas estratégias corroboram a perseverança dos cabo-verdianos em encontrar os cuidados de saúde que melhor se adequam às suas necessidades.

Chronic illness is itself a significant cause of worklessness and low pay. Drawing on examples from different countries, this book shows that the relationship between work, worklessness and health varies by country. Countries with a more regulated work environment and a more interventionist and supportive welfare system have better health and smaller work-related health inequalities. The book provides examples of specific policies and interventions that mitigate the ill-health effects of work and worklessness. It concludes by asserting the importance of politics and policy choices in the aetiology of health and health inequalities.

Book excerpt: In the 1950s–80s, Brazil built one of the most advanced industrial networks among the "developing" countries, initially concentrated in the state of São Paulo. But from the 1980s, decentralization of industry spread to other states reducing São Paulo's relative importance in the country's industrial product. This volume draws on social, economic, and demographic data to document the accelerated industrialization of the state and its subsequent shift to a service economy amidst worsening social and economic inequality. Through its cultural institutions, universities, banking, and corporate sectors, the municipality of São Paulo would become a world metropolis.
