Book excerpt: Ce travail est issu de notre expérience d'interne en Équipe Mobile de Psychiatrie et Précarité du CHU de Saint-Étienne, auprès des personnes sans domicile fixe (SDF). Les études épidémiologiques révèlent une surmorbidité psychiatrique dans cette population. Nous nous référons aux théories anthropologique, sociale et psychopathologique pour explorer le phénomène de la grande exclusion afin d'améliorer la compréhension et la prise en charge de ces personnes. La complexité de la clinique ne permet pas de définir une causalité linéaire entre le phénomène SDF et la maladie mentale mais les symptômes de la désocialisation évoquent ceux de la psychose et des troubles graves du narcissisme, comme l'illustre l'analyse du suivi d'un ancien SDF. À la lumière de cette expérience, nous faisons l'hypothèse que le psychiatre trouve sa place dans l'analyse des modalités d'intervention des travailleurs sociaux de première ligne. Dans l'entre-deux du travail social et de l'institution hospitalière, le cadre thérapeutique doit garantir un espace d'élaboration et de créativité.
