Il s'agit d'un processus de destruction par des odontoclastes des structures dures de la dent (émail, dentine, cément) pouvant progresser et entraîner une perforation radiculaire. Il existe trois grands types de résorptions externes : Inflammatoire, de remplacement et de surface. Ces résorptions peuvent avoir lieu au niveau radiculaire aussi bien cervicalement qu'amicalement. Elles ont une origine variée pouvant faire suite à un processus infectieux, un traumatisme ou d'origine iatrogène comme lors d'un traitement orthodontique. Lorsqu'aucune cause n'est identifiée il s'agit alors de résorptions idiopathiques. Le diagnostic des résorptions externes est trop souvent tardif, elles sont habituellement asymptomatiques et le plus souvent détectées lors d'examens radiographiques ou cliniques de routine. Les signes cliniques apparaissent lorsque la lésion a progressé à un stade avancé pouvant entraîner une inflammation du parodonte marginal, ou lorsque que le système canalaire est infecté entraînant alors des symptômes de parodontite apicale aigüe. Le diagnostic précoce reposera en majeure partie sur la radiographie (la radiographie rétro alvéolaire en première intention et la tomodensitométrie à faisceau conique) et qui sera un outil précieux dans la détection de ces lésions. En ce qui concerne le traitement de ces résorptions il pourra être préventif ou curatif, allant de traitements non chirurgicaux comme le traitement canalaire, avec utilisation de matériaux comme l'hydroxyde de calcium ou le MTA (Mineral trioxide aggregate), jusqu'à des techniques chirurgicales comme l'élévation d'un lambeau, l'extrusion orthodontique ou l'extraction/réimplantation. Malheureusement certains cas échappent encore à nos techniques actuelles nous contraignant à une abstention thérapeutique ou à l'avulsion d'une dent ne pouvant pas être conservée, mais l'évolution des biomatériaux mis à notre disposition et l'apparition de nouvelles techniques comme l'irradiation laser nous mènent vers une avancée constante dans le traitement des résorptions externes pathologiques.

Ces résorptions s’initient suite à la lésion des structures parodontales et l’exposition des tubuli dentinaires et sont entretenues par les toxines bactériennes issues de la nécrose pulpaire. Le traitement de ces résorptions repose donc sur l’élimination des bactéries endopathogènes du système canalaire. Actuellement, le traitement de référence passe par un traitement intracanalaire à l’hydroxyde de calcium de longue durée. Bien que le taux de succès soit élevé, cette thérapeutique présente de nombreux inconvénients, dont la durée de traitement et le risque important de fracture radiculaire secondaire. L’émergence de nouveaux biomatériaux à base de silicate de calcium présente un intérêt majeur dans les thérapeutiques endodontiques. Un suivi de cas cliniques de patients présentant des résorptions post-traumatiques avérées, traitées avec la BiodentineTM, permet de valider l’intérêt de ces ciments dans le traitement des résorptions radiculaires externes inflammatoires.
