Book excerpt: Contexte : La prévalence du diabète de type 2 traité pharmacologiquement en France métropolitaine est estimée à 3,95% en 2007. En février 2013, une nouvelle recommandation de l'HAS est sortie sur la prise en charge des diabétiques de type 2. Elle est centrée sur le patient. L'objectif de cette étude est de recueillir le ressenti des médecins généralistes sur la personnalisation de la prise en charge des diabétiques de type 2 et de mettre en évidence les motifs pouvant les amener à diminuer le traitement médicamenteux. Méthode : cette enquête qualitative a été menée auprès de 15 médecins généralistes du Nord par des entretiens semi-dirigés. Elle a été conduite jusqu'à saturation des données. Un codage des données a été réalisé par théorisation ancrée sur le logiciel N-vivo10 avec triangulation des codes ouverts. Résultats : Il en ressort que les médecins généralistes appliquaient déjà les directives de la recommandation HAS 2013 bien avant sa sortie. Ils perçoivent, dans la personnalisation du traitement du diabétique de type 2, un ensemble de critères d'adaptation à la fois clinico-biologiques et socio-économiques. Ces derniers sont également les causes pouvant les amener à baisser les antidiabétiques. Dans les deux cas, ils évoquent de nombreuses difficultés à y parvenir. Conclusion : Les résultats obtenus ne font que confirmer la problématique de la prise en charge des diabétiques de type 2 par les médecins généralistes malgré les recommandations de bonnes pratiques récentes. La perspective de directives plus précises sur les motifs de diminution des traitements pourraient apporter une aide supplémentaire.

INTRODUCTION : Le diabète de type 2 est une maladie chronique dont le traitement repose autant sur l'application de règles hygiéno-diététiques (alimentation, activité physique, régulation du poids) que les médicaments. Il semble qu'en France la part médicamenteuse soit plus importante que chez nos voisins européens. MATERIELS ET METHODES : Il s'agit d'une étude quantitative prospective transversale. 200 questionnaires ont déposés à la consultation diabète de l'hôpital de Fréjus et dans 10 cabinets de médecins généralistes des villes de Fréjus-Saint Raphaël (Var, 83). RESULTATS : Le taux de réponse a été de 21% (42/200). 42 patients ont été inclus dans l'étude. Il s'agissait majoritairement de femmes (54,1%), d'âge moyen de 64 ans et d'IMC moyen de 30,19 kg/m2. La majorité de ces patients a déclaré avoir reçu une information à propos de la prise en charge non médicamenteuse du diabète. Le généraliste est apparu comme la première source d'information (80,5%). L'information était majoritairement délivrée par oral (72,7 %) et jugée pertinente par 82,7 % des patients. 42,9 % des patients avaient un bon niveau de connaissances de leur pathologie, et 54 % un très bon niveau d'application des recommandations hygiéno-diététiques. Les deux principaux obstacles freinant l'application de ces mesures étaient la lassitude et la prise des repas à l'extérieur. Le généraliste était jugé compétent pour le suivi de la maladie diabétique par 83,3 %, et 68,3% ont souhaité bénéficier d'aide(s) supplémentaire(s) pour la gestion de leur maladie au quotidien, dont le principal était la consultation diététique. CONCLUSION : La prise en charge non médicamenteuse du patient diabétique de type 2 à Fréjus-Saint Raphaël est globalement satisfaisante, d'après les patients. Les recommandations sont plutôt bien appliquées dans la vie quotidienne. Le médecin traitant tient une place prédominante. Les mesures visant à améliorer l'éducation thérapeutique des patients diabétiques de type 2 doivent être poursuivies et renforcées.
