Book excerpt: Les troubles impliquant la souffrance psychique (SP) ont une forte prévalence dans la population générale et sont pris en charge à 80% par les médecins généralistes. Le concept de SP peut être défini comme un état de mal être ne relevant pas forcément d’une pathologie mentale. Il émerge sur un terrain fragilisé (médical, familial, professionnel, social) et est souvent déclenché par des événements de vie. L'objectif de notre travail est de recueillir les représentations que les médecins généralistes se font de la souffrance psychique de leur patient. 24 généralistes ont participé à des entretiens individuels semi-dirigés en Maine et Loire, Sarthe et Tarn. Une double analyse thématique a été effectuée à partir des verbatim. Ce travail objective le rôle central du médecin généraliste. Utilisant le modèle bio-psycho-social, il peut désamorcer les situations de SP. Il s’est constitué un savoir pratique qu'il adapte à chaque patient. Il détecte la SP par une perception subjective, et renvoie un message de compréhension. Écoute et empathie sont les piliers de sa prise en charge. Il questionne la place des médicaments et des spécialistes en santé mentale. La SP du patient peut générer une souffrance chez le médecin pour laquelle beaucoup développent des stratégies. Ce travail suggère une amélioration de la formation initiale : théorique avec acquisition de repères psycho-sociaux, et pratique par la confrontation précoce des étudiants à ces situations, amenant à une réflexion en groupe d’échange et d’analyse de pratique.

Histories, novels, and films have cloaked his life in hagiography and legend. Jacalyn Duffin's fascinating new biography relies on a vastly expanded foundation of primary source material, including thousands of pages of handwritten patient records, lecture notes, unpublished essays, and letters. She situates Laennec, the scientist and teacher, within the broader social and intellectual currents of post-Revolutionary France. Her work uncovers a complex character who participated actively in the dramatic changes of his time. Laennec's famous Treatise on Mediate Auscultation was his only published book, but two lesser known works were left in manuscript: an early treatise on pathological anatomy and a later set of lectures on disease. The three parts of Duffin's biography correspond to these books. First, she examines Laennec's student research on the emerging science of pathological anatomy, the background for his major achievement. Second, she uses his clinical records to trace the discovery and development of "mediate auscultation" (listening through an instrument, or mediator, to sounds within the human body). The stethoscope allowed clinicians to "see" the organic alterations inside their living patients' bodies. Finally, she explores the impact of auscultation on diagnostic practice and on concepts of disease. Analyzed here for the first time in their entirety, Laennec's Collége de France lectures reveal his criticism of over-enthusiastic extrapolations of his own method at the expense of the patient's story. The Cognitive Behavioural Tsunami: Managerialism, Politics, and the Corruptions of Science provides a powerful critique of CBT’s understanding of human suffering, as well as the apparent scientific basis underlying it. The book argues that CBT psychology has fetishized measurement to such a degree that it has come to believe that only the countable counts. It suggests that the so-called science of CBT is not just "bad science" but "corrupt science". The rise of CBT has been fostered by neoliberalism and the phenomenon of New Public Management. The book not only critiques the science, psychology and philosophy of CBT, but also challenges the managerialist mentality and its hyper-rational understanding of "efficiency", both of which are commonplace in organizational life today. The book suggests that these are perverse forms of thought, which have been institutionalised by NICE and IAPT and used by them to generate narratives of CBT’s prowess. It claims that CBT is an exercise in symptom reduction which vastly exaggerates the degree to which symptoms are reduced, the durability of the improvement, as well as the numbers of people it helps. Arguing that CBT is neither the cure nor the scientific treatment it claims to be, the book also serves as a broader cultural critique of the times we live in; a critique which draws on philosophy and politics, on economics and psychology, on sociology and history, and ultimately, on the idea of science itself. Nevertheless, this report also identifies weaknesses in Europe, some systematic, such as a lack of consensus on definitions, and of compatible data collection, and others such as the need for development and investment across several areas. It identifies gaps in information where further work must be done, and is a baseline against which progress can be measured towards the vision and the milestones of the Mental Health Declaration for Europe. The data were obtained from the ministries of health of 42 European Member States. Over 150 figures and tables in the report demonstrate the diversity across the European Region, and allow country to country comparisons of indicators such as numbers of psychiatrists, financing, community services, training of the workforce, the prescription of anti-depressants and representation of users and carers.
