Quelles sont les répercussions de l'utilisation de la grille de Zarit sur la prise en charge de l'aidant et du patient dément en médecine générale ? Revue systématique de la littérature effectuée à partir de 8 bases de données, de recherches manuelles et des bibliographies des articles retenus. Les articles concernant les déterminants du Zarit, ceux utilisant le Zarit comme critère de jugement de l'efficacité d'une intervention et ceux concernant d'autres types de démence ont été exclus. 12 articles ont été inclus. Un score de Zarit élevé est associé à un risque augmenté d'hospitalisation, d'institutionnalisation et de maltraitance du patient dément ainsi qu'à un risque d'altération psychopathologique de l'aidant. Un fardeau élevé persistant est un facteur prédictif de dépression de l'aidant. L'analyse de données venant de l'aidant (autonomie du patient, qualité de vie) doit être interprétée selon son niveau de fardeau. L'utilisation du Zarit permet de formaliser le dépistage du fardeau de l'aidant et de repérer les dyades à risque. La validation du mini-Zarit, la réalisation d'études en soins primaires et la prise en compte des conséquences positives de l'aide sont des pistes pour améliorer la prise en charge des patients déments et de leurs aidants.

Pour évaluer le fardeau, l'outil le plus couramment utilisé est l'échelle de Zarit. L'objectif de cette étude est d'étudier le ressenti d'aidants familiaux de malade d'Alzheimer lors de l'utilisation de l'échelle de Zarit en consultation de médecine générale ainsi que les intérêts et les limites de cette échelle. Méthode Une étude qualitative par la technique des focus groups a été réalisée. Les aidants inclus étaient les aidants principaux de malades d'Alzheimer. Une analyse thématique a été effectuée. Résultats 19 aidants ont été interrogés lors de 4 focus groups. Les aidants étaient majoritairement enthousiastes à l'utilisation de l'échelle de Zarit. Ils lui ont trouvé des intérêts : pour l'aidant, pour le médecin et la prise en charge. Des points négatifs étaient retrouvés. L'échelle était décrite trop longue et complexe dans sa réalisation. Les conditions de sa réalisation et son intérêt étaient débattues. Discussion L'échelle de Zarit est un outil perfectible pour évaluer et suivre le fardeau des aidants de malades d'Alzheimer, de même que les autres échelles de fardeau recensées dans la littérature. L'évaluation du fardeau uniquement sous ses aspects négatifs est restrictive et source de conséquences négatives. Il paraît nécessaire de repenser l'utilisation de l'échelle. La collaboration entre professionnels de santé devrait permettre d'utiliser au mieux l'échelle lors de consultations dédiées aux aidants.

Book excerpt: L'échelle Zarit (Caregiver Burden Scale ou Zarit Burden Inventory) a été développée en 1980 pour mesurer le fardeau de l'aidant de patient atteint de maladie d'Alzheimer. Son utilisation est recommandée en médecine générale pour dépister ce fardeau. L'échelle Zarit contient ainsi des questions pertinentes dans le contexte de la maladie d'Alzheimer et explore par exemple l'embarras que l'aidant pourrait ressentir par rapport aux troubles du comportement de son proche. Cependant, cette échelle est utilisée pour mesurer le fardeau d'aidants de patients atteints d'autres pathologies ou d'autres situations pour lesquelles les ques tions de l'échelle Zarit pourraient manquer de pertinence. OBJECTIF : L'objectif principal de ce travail était d'analyser la diffusion dans la littérature scientifique de cette échelle dans le temps et les situations médicales. METHODE : Revue de littérature réalisée à travers les bases de données Medline, Cochrane, Cismef, Sciencedirect et Em-premium de 1980 à septembre 2017. Plusieurs équations de recherche ont été utilisées avec en commun les termes Zarit et Pathology selon les [ MeSH Terms]. Seuls les articles princeps de chaque article publié ont été retenus pour l'analyse ; les articles exposant les résultats d'une analyse ancillaire ou secondaire ont été exclus après analyse du résumé, puis du texte intégral et de la bibliographie en cas de doute. Le recueil de données a été réalisé à l'aide d'une fiche élaborée par consensus des trois chercheurs et testée sur trois articles. Les données r ecueillies ont été la base de données, le titre, les noms des auteurs, l'année de publication, le nom de la revue , la spécialité médicale et la version du questionnaire Zarit utilisée. L'analyse des résultats était descriptive avec la réalisation graphique d'une flèche chronologique et d'un mind mapping. RESULTATS : 45 articles princeps ont été inclus correspondant à l'étude du fardeau de l'aidant dans 45 pathologies. L'utilisation de l'échelle de Zarit a diffusé dans les différentes situations médicales à partir de 1995. Les deux spécialités médicales les plus représentées étaient la neurologie et la psychiatrie. Plu sieurs mécanismes de diffusion ont été mis en évidence tels que des mécanismes directement liés au Pr Steven H. Zarit, et les autres comprenant l'utilisation de l'échelle pour son critère de validité linguistique et culturelle, les auteurs en commun entre les différents articles et les programmes d'études européens. CONCLUSION : L'échelle de Zarit a été utilisée à partir de 1995 pour évaluer le fardeau de l'aidant dans des situations médicales différentes des pathologies démentielles pour lesquelles la pertinence des items serait à étudier.
