Book excerpt: Du fait de leurs fréquentes polypathologies entrainant une polymédication, de leurs modifications physiologiques et des modifications pharmacocinétiques et pharmacodynamiques, les personnes âgées sont plus exposées à la iatrogénie médicamenteuse, enjeu majeur de santé publique. Certains médicaments prescrits peuvent être potentiellement inappropriés et majorer le risque de survenue d'effets indésirables. L'objectif de notre étude était d'évaluer la prévalence des prescriptions potentiellement inappropriées (PPI) au sein des prescriptions médicamenteuses de ville et d'évaluer l'impact d'une hospitalisation dans un service de gériatrie aigue sur ces prescriptions. Cette étude observationnelle, rétrospective a été réalisée au sein d'une population de sujets âgés de 75 ans et plus, hospitalisés dans le service de court séjour gériatrique du centre hospitalier de Libourne, en Mars 2015. Pour chaque patient, nous avons analysé l'ordonnance à l'admission et l'ordonnance à la sortie de l'hospitalisation selon deux listes de détection de PPI validées : la liste de Laroche et la version française de STOPP-START. Nous avons ainsi relevé les médicaments identifiés comme inappropriés selon Laroche et STOPP et omis selon START. Au total, 180 patients ont été inclus. Ils avaient en moyenne 6,76 médicaments à l'entrée et 6,82 à la sortie. 70,6% des patients avaient des PPI à l'admission et 53,3% en avaient à la sortie. Les classes thérapeutiques impliquées étaient les antiagrégants plaquettaires, les benzodiazépines et les inhibiteurs de la pompe à protons. L'hospitalisation avait un impact favorable en réduisant de 24,4% le nombre d'ordonnances sans PPI et en diminuant la prescription inappropriée de 35,6%. L'optimisation de la prescription médicamenteuse chez la personne âgée doit être poursuivie et régulièrement réévaluée: la diffusion de STOPP-START auprès des médecins! génér alistes et la collaboration étroite avec les pharmaciens semblent être des solutions à explorer.

L'objectif principal de l'étude est la mise en évidence d'une différence significative entre l'information donnée par le pharmacien et celle donnée par le médecin généraliste concernant le nombre de médicaments et les classes thérapeutiques auxquels le patient est exposé. Méthode : Cette étude transversale et prospective a inclus les patients de 75 ans et plus hospitalisés. Le médecin traitant et le pharmacien habituel étaient contactés simultanément afin de recueillir la liste des médicaments prescrits et délivrés les 3 derniers mois. Résultats : L'information du médecin traitant et du pharmacien ont été obtenues pour 11 patients. Le nombre de médicaments prescrits et délivrés est différent pour 10 patients sur 11 (90,9% ; p=0,04) et pour 10 patients sur 11 (90,9%) en termes de classes thérapeutiques. Les médicaments issus de la divergence des ordonnances sont : médicaments à visée cardio-vasculaire (81,8%), antibiotiques (45,2%), inhibiteurs de la pompe à protons (27,2%), benzodiazépines (18,2%) et antalgiques de palier 2 (18,2%). Les personnes âgées, souvent polypathologiques, sont de grandes consommatrices de médicaments. La question se pose donc de la qualité de leurs prescriptions.Face à cette situation, des outils d’aide à la prescription se sont développés, en particulier aux Etats-Unis et au Canada, avec élaboration de listes de médicaments à éviter. La liste de Médicaments Potentiellement Inappropriés (PIMs) chez les sujets âgés de 75 ans et plus, établie à la suite de ces travaux par ML Laroche, est particulièrement intéressante, puisqu’elle s’applique à la pratique médicale française. Une étude de la fréquence de prescription des PIMs a été réalisée au CHU d’Angers à partir de cette liste française. Elle présente des résultats alarmants : 42,3% des patients étudiés ont au moins un PIM dans leur prescription. Les benzodiazépines sont les molécules les plus incriminées. Ces résultats sont cohérents avec les autres études effectuées dans le même domaine. Ces observations mettent en évidence la nécessité d’accroître la vigilance du personnel de santé lors de la prescription et la délivrance de médicaments dans la population âgée. Dans ce cadre, la diffusion d’une telle liste pourrait aider à promouvoir la qualité des traitements du sujet âgé.
