Book excerpt: Contexte : La prise en charge thérapeutique de la ménopause, à plus de dix ans du cataclysme provoqué par l'étude américaine WHI, reste difficile en médecine générale. L'objectif de cette étude était d'en évaluer les freins aujourd'hui. Méthodes : Etude qualitative, basée sur des entretiens semi-directifs individuels auprès de vingt médecins généralistes dans le Nord-Pas-De-Calais en 2013. Un échantillonnage en variation maximale a été réalisé. L'analyse des résultats a été faite en double aveugle avec le logiciel NvivolO, par une approche thématique. Résultats : Les résultats de l'étude WHI ont eu un impact fort chez les médecins généralistes car ils étaient toujours cités comme principal frein au traitement hormonal de la ménopause (THM) aujourd'hui. L'appréhension du risque cardiovasculaire et du cancer du sein était importante. Les bénéfices du THM, eux, étaient jugés limités ou au second plan lorsqu'il s'agissait de l'ostéoporose post ménopausique. Les recommandations et les connaissances sur le THM étaient jugées insuffisantes : elles entraînaient difficultés et perte de repère vis-à-vis du traitement. Le recours au spécialiste gynécologue était largement utilisé et considéré comme préféré par les patientes. Les conséquences de la carence estrogénique étaient sousestimées ou perçues comme naturelles. Enfin, des difficultés de prescription (schéma, durée limitée) et la réticence des patientes étaient aussi retrouvées comme obstacle au THM. Conclusion : Les médecins généralistes éprouvent aujourd'hui des difficultés face au THM, conséquences directes et indirectes de l'étude WHI. Chaque point pouvant cependant être repris pour améliorer la prise en charge de la femme ménopausée.

Do we over-generalize in conceptualizing what is unrepresented, with the corresponding problem of false positives? Do dreams equal understanding and what about the generalizability of the co-created reverie? Busch concludes that it is primarily through the analyst’s own associations that the reverie’s potential is revealed, which further helps the analyst distinguish it from many other possibilities, including the analyst’s countertransference. He believes in the importance of converting reveries into verbal interpretations, a controversial point amongst post-Bionians. Busch ends with the difficult task of classifying the analyst’s reveries based on their degree of representation.
