Pour autant, il semble que ce geste soit pratiqué dans le secret, notamment à domicile chez des patients en fin de vie, par certains médecins généralistes. La question de l'euthanasie soulève de nombreuses polémiques notamment à propos d'une éventuelle légalisation. L'objectif de ce travail de recherche était de comprendre les raisons de ce geste, et d'en déduire des facteurs déterminants de l'acte d'euthanasie. Ainsi, un travail de recherche qualitatif a été réalisé, au moyen d'entretiens semi-dirigés, anonymes, chez des médecins généralistes choisi au hasard sur l'annuaire. Quatorze facteurs déterminants ont été retrouvés, dont cinq facteurs déterminants concrètement modifiables, et six issus d'une approche non rationnelle dans la prise de décision. Les cinq facteurs déterminants concrètement modifiables (1 /ne pas faire appel aux équipes mobiles de soins palliatif, 2/I'ignorance de la loi Leonetti, 3/de la notion d'intentionnalité dans la définition de l'euthanasie, 4/le manque de connaissance dans l'utilisation des antalgiques et autres traitements palliatifs,5/ la solitude et le manque de collégialité) pourraient faire l'objet d'une prévention de l'acte d'euthanasie en médecine générale. L'euthanasie résulte donc de nombreux facteurs, dont certains sont concrètement modifiables, et d'autres non rationnels, ce serait par conséquent une erreur de la légaliser.

En réponse à ce problème, certains pays ont légiféré sur la dépénalisation de l'euthanasie et/ou suicide assisté. En France, ce sujet est à l'origine de vifs débats opposants ses partisans et ses adversaires. Le médecin généraliste accompagne et occupe un rôle de premiers recours pou le patients en fin de vie et leurs familles. L'objectif de ce travail est donc de rechercher les facteurs qui déterminent l'opinion des médecins généralistes sur la dépénalisation de l'euthanasie et du suicide assisté. Pour cela, nous avons mené une étude qualitative à l'aide d'entretiens semi-dirigés auprès d'un échantillon de médecins généralistes du département de l'Aude. Une multitude de facteurs déterminants ont été mis en évidence. Nous constatons que les médecins s'appuient très largement sur leurs expériences professionnelles et sur la loi Léonetti pour justifier leurs positions. De plus, une majorité de médecins généralistes se prononce contre la dépénalisation de l'euthanasie et du suicide assisté. Car ils jugent ces questions exceptionnelles voir absentes dans leur pratique quotidienne. L'euthanasie et le suicide assisté sont des sujets qui engendrent un grand nombre d'interrogations sur notre société. Il est donc nécessaire de continuer de débattre sur la fin de vie, afin d'en améliorer la prise en charge et de répondre au mieux aux attentes de nos patients

Book excerpt: This paper argues that oil revenue management and public investment in Congo are vulnerable to corruption as a result of limited transparency and accountability. Corruption has potentially contributed to poor macro-fiscal outcomes. The paper acknowledges the authorities’ anti-corruption efforts made so far and proposes further critical reforms to reduce remaining vulnerabilities. Using a dynamic stochastic general equilibrium model results show that, depending on the reforms adopted, the potential additional growth can range between 0.8 to 1.8 percent per year over the next 10 years, and debt can decline by 2.25 to 3 percent of GDP per year over the same period. These results suggest that macrofiscal gains from anti-corruption reforms could be substantial even under conservative reform scenarios.
