Book excerpt: La mission des Équipe Mobile Liaison Psychiatrie Précarité - Plate-forme Accès Soins Santé (EMPP-PASS) consiste en l'orientation du public « précaire » vers les dispositifs de droit commun et l'accompagnement des équipes soignantes dans leur prise en charge. En psychiatrie, les soins sectorisés rendent leur accès plus complexe au public sans domicile fixe, ce qui ne respecte pas toujours les règles de la continuité des soins du secteur. Notre étude a pour objectif d'explorer la pertinence du protocole d'admission des patients SDF au centre hospitalier Alpes-Isère en vue d'en proposer des aménagements pour une meilleure continuité des soins en hospitalisation et pour un accompagnement plus adapté des patients et des équipes soignantes. Nous étudions dans un premier temps les besoins de ces patients à travers les demandes qu'ils font à l'EMPP-PASSpsy de Grenoble en utilisant la technique qualitative de consensus du Groupe Nominal. Les résultats montrent que la question du sens de l'hospitalisation est primordial : un travail en réseau pour une meilleure cohérence du projet de soins est indispensable ainsi qu'un effort d'information tant des équipes que des patients, avec interprète s'il le faut. Les résultats montrent aussi que observation clinique et besoins fondamentaux sont des priorités en lien avec la particulière vulnérabilité de ce public. Dans un second temps, nous discutons les résultats au regard de la littérature ce qui nous permet de mettre en lumière les dysfonctionnements actuels de l'admission des SDF à l'hôpital. Cela nous autorise à recommander certains aménagements comme l'affiliation définitive de chaque patient SDF à un pôle de référence et une prise en charge institutionnelle en réseau portée par l'EMPP.
