Por lo mismo, el tiempo que ha de dedicarse a cada paciente es mayor que el de las adaptaciones convencionales. Se requiere conocer el perfil corneal, de manera cualitativa y cuantitativa, y por lo mismo es necesario apoyarse en topografía corneal convencional o Plácido, o de elevación o tomografía corneal.

La primera trata sobre la identificación del tipo de queratocono, analizando el tamaño de la ectasia, para seleccionar, de acuerdo con esta, el diseño de la curva base y el diámetro. La segunda, la selección de los parámetros principales de adaptación de lentes rígidos de curva base esférica para pacientes con córneas irregulares posteriores a cirugía refractiva de tipo láser.

Cuando las córneas son irregulares debido a una deformación en su cara anterior —la más conocida por todos, el queratocono—, la adaptación de los lentes se complica y debe realizarse con ayuda de aparatos que ofrezcan información de todo el perfil corneal, como los topógrafos o tomógrafos corneales. La cirugía refractiva es en general un gran logro de la oftalmología, y Colombia es uno de los países en donde se tienen mayores desarrollos sobre este caso particular. Algunas veces, por inconvenientes en la adecuada selección del paciente, problemas en el desarrollo de la cirugía u otro tipo de problemas posteriores, quedan pacientes con córneas irregulares que no logran buena visión con anteojos y que deben ser adaptados con algún diseño especial de lentes de contacto de apoyo corneal o escleral. Para estas adaptaciones es muy importante la adecuada interpretación de las imágenes topográficas y tomográficas de cornea.

Nailfold videocapillaroscopy (NVC) represents the safest, noninvasive, less expensive and reliable method to analyze microvascular abnormalities in systemic sclerosis and the findings can be scored and quantified. Since microvascular damage and dysfunction represent early markers of systemic sclerosis and are clinically mirrored by secondary Raynaud phenomenon, the diagnostic, prognostic and therapeutic implications of microvessel morphological analysis by NVC enables the best clinical management. Reduced capillary density on NVC predict and correlates with a high risk of developing digital skin ulcers and the presence of pulmonary arterial hypertension, and can therefore be used as a marker of systemic sclerosis severity and progression, as well as to monitorize the therapeutical effects. Growing interest and intense recent research on capillaroscopy have resulted up to 2010 in almost 350 papers reported in PubMed in the last 10 years, accounting for almost 50% of the total number of publications on the subject from 1951. However, an atlas explaining the capillaroscopy in rheumatic diseases in a modern, updated and physiologically-based way with large discussion of clinical cases was lacking and is now available. The authors of the atlas have been selected among the best international experts of the field and are all members of the board of the international courses on capillaroscopy supported by the European Ligue Against Rheumatic Diseases (EULAR).
