Book excerpt: From the mid-nineteenth to the late twentieth centuries, Saint Elizabeths Hospital was one of the United States' most important institutions for the care and treatment of the mentally ill. Founded in 1855 to treat insane soldiers and sailors as well as civilian residents in the nation's capital, the institution became one of the country's preeminent research and teaching psychiatric hospitals. From the beginning of its operation, Saint Elizabeths admitted black patients, making it one of the few American asylums to do so. This book is a history of the hospital and its relationship to Washington, DC's African American community. It charts the history of Saint Elizabeths from its founding to the late-1980s, when the hospital's mission and capabilities changed as a result of deinstitutionalization, and its transfer from the federal government to the District of Columbia. Drawing on a wide variety of sources, including patient case files, the book demonstrates how race was central to virtually every aspect of the hospital's existence, from the ways in which psychiatrists understood mental illness and employed therapies to treat it to the ways that black patients experienced their institutionalization. The book argues that assumptions about the existence of distinctive black and white psyches shaped the therapeutic and diagnostic regimes in the hospital and left a legacy of poor treatment of African American patients, even after psychiatrists had begun to reject racialist conceptions of the psyche. Yet black patients and their communities asserted their own agency and exhibited a "rights consciousness" in large and small ways, from agitating for more equal treatment to attempting to manage the therapeutic experience.

Book excerpt: A look into communicating psychiatric patient histories, from the asylum years to the clinics of today In this engrossing study of tales of mental illness, Carol Berkenkotter examines the evolving role of case history narratives in the growth of psychiatry as a medical profession. Patient Tales follows the development of psychiatric case histories from their origins at Edinburgh Medical School and the Royal Edinburgh Infirmary in the mid-eighteenth century to the medical records of contemporary American mental health clinics. Spanning two centuries and several disciplines, Berkenkotter's investigation illustrates how discursive changes in this genre mirrored evolving assumptions and epistemological commitments among those who cared for the mentally ill. During the asylum era, case histories were a means by which practitioners organized and disseminated local knowledge through professional societies, affiliations, and journals. The way in which these histories were recorded was subsequently codified, giving rise to a genre. In her thorough reading of Sigmund Freud's Fragment of an Analysis of a Case of Hysteria, Berkenkotter shows how this account of Freud's famous patient "Dora" led to technical innovation in the genre through the incorporation of literary devices. In the volume's final section, Berkenkotter carries the discussion forward to the present in her examination of the turn from psychoanalysis to a research-based and medically oriented classification system now utilized by the American Psychiatric Association. Throughout her work Berkenkotter stresses the value of reading case histories as an interdisciplinary bridge between the humanities and sciences.

Available in PDF, EPUB and Kindle. Book excerpt: The asylum--at once a place of refuge, incarceration, and abuse--touched the lives of many Americans living between 1830 and 1950. What began as a few scattered institutions in the mid-eighteenth century grew to 579 public and private asylums by the 1940s. About one out of every 280 Americans was an inmate in an asylum at an annual cost to taxpayers of approximately $200 million. Using the writing of former asylum inmates, as well as other sources, Writing Mad Lives in the Age of the Asylum reveals a history of madness and the asylum that has remained hidden by a focus on doctors, diagnoses, and other interventions into mad people's lives. Although those details are present in this story, its focus is the hundreds of inmates who spoke out or published pamphlets, memorials, memoirs, and articles about their experiences. They recalled physical beatings and prolonged restraint and isolation. They described what it felt like to be gawked at like animals by visitors and the hardships they faced re-entering the community. Many inmates argued that asylums were more akin to prisons than medical facilities and testified before state legislatures and the US Congress, lobbying for reforms to what became popularly known as "lunacy laws." Michael Rembis demonstrates how their stories influenced popular, legal, and medical conceptualizations of madness and the asylum at a time when most Americans seemed to be groping toward a more modern understanding of the many different forms of "insanity." The result is a clearer sense of the role of mad people and their allies in shaping one of the largest state expenditures in the nineteenth and early twentieth centuries--and, at the same time, a recovery of the social and political agency of these vibrant and dynamic "mad writers."
