L'évaluation de l'organisation et de la charge de travail du généraliste constitue un réel enjeu pour l'amélioration de la qualité des soins primaires. Les objectifs de notre étude étaient de quantifier la charge de travail administratif pendant les consultations des médecins généralistes libéraux des Alpes-Maritimes, d'analyser ses déterminants et de proposer des pistes d'optimisation de l'exercice libéral. Nous avons réalisé une étude observationnelle sur des généralistes libéraux. Chacun a été observé sur une demi-journée de travail, pendant laquelle les tâches administratives réalisées per consultation étaient relevées et chronométrées. Au total, 674 consultations de 51 médecins, représentatifs des généralistes des Alpes-Maritimes, ont été analysées. Le temps de consultation total était de 166 h 13 min, soit une durée de consultation moyenne de 14 min 47 s. 24 h 50 min étaient consacrées au travail administratif soit 15 % du temps de consultation et plus de 4 h de travail hebdomadaire. Une consultation sur deux comprenait au moins 3 tâches. Les feuilles de soins concernaient 99 % des consultations, soit 59 minutes hebdomadaires. Une consultation sur trois comportait des tâches de secrétariat, la gestion des rendez-vous occupant 30 minutes par semaine. 24,8 % des consultations étaient interrompues par des appels téléphoniques, représentant 51 minutes hebdomadaires. La rédaction de certificats et formulaires divers intéressait 47 % des consultations et occupaient 1 h 30 min par semaine. Les médecins sans secrétariat faisaient significativement plus de tâches administratives, principalement des tâches de secrétariat. Ils étaient interrompus 7 fois plus que ceux employant une secrétaire et 2 fois plus que ceux utilisant un télésecrétariat. Le travail administratif constitue un réel fardeau pour les généralistes, dégrade leur condition de travail et entrave la qualité des soins par leur fréquence et leur poids horaire. Valoriser la médecine libérale par une sensibilisation des futurs généralistes, une facilitation de l'accès à une assistance administrative et une démarche d'optimisation de l'efficience des cabinets sont essentielles pour construire le modèle libéral de demain.
