La recherche d'une HTA secondaire est recommandée chez les patientes hypertendues de moins de 40 ans mais les modalités de dépistage du SAOS ne sont pas précisées. L'objectif principal de notre étude était de déterminer la prévalence du SAOS chez les femmes jeunes hypertendues et d'évaluer la pertinence d'un dépistage systématique par polysomnographie (PSG) ou polygraphie ventilatoire (PG). L'objectif secondaire était de préciser les facteurs associés au SAOS dans cette population. Méthode : Une étude observationnelle, transversale a été menée. Toutes les femmes de moins de 40 ans hospitalisées pour bilan d'HTA dans le service d'HTA et médecine vascuiaire du CHRU de Lille ont été convoquées pour réalisation d'une PG ou PSG. Résultats : Parmi les 59 patientes convoquées, 37 ont réalisé leur PG ou PSG. Un SAOS a été diagnostiqué chez 18,92% des femmes jeunes hypertendues. Plus de 70% des SAOS étaient légers. Comparées aux patientes sans SAOS, les femmes ayant un SAOS rapportaient plus souvent des ronflements (100% vs 42,86% ; p=0,0092), des apnées (28,57% vs 0% • p=0,0353) et une somnolence diurne excessive définie par un score d'Epworth >10 (57,14% vs 13,33% ; p=0,0271). L'IMC des patientes ayant un SAOS était supérieur à celui des patientes sans SAOS (40,10 kg/m2 vs 26,45 kg/m2 ; p=0,0138). Le seuil d'IMC le plus discriminant vis à vis du SAOS dans notre population était 35 kg/m2. Cinq patientes du groupe SAOS avaient un IMC > 35 kg/m2 soit 71,43% contre seulement 16,67% des femmes n'ayant pas de SAOS (p=0,0092) Conclusion : Bien que le SAOS ne soit pas rare chez les femmes jeunes hypertendues, il n'est pas pertinent de proposer un dépistage systématique par PG ou PSG. Les critères devant motiver la réalisation de ces examens sont : les ronflements, les apnées, la somnolence diurne excessive et un IMC > 35 kg/m2.
