Le médecin généraliste a un rôle à jouer mais peine parfois à trouver sa place. La politique nationale de santé met en avant la E-santé et notamment la télésurveillance dans le cadre du diabète. Objectif : à travers l'exploration et la compréhension des difficultés que ressentent les généralistes dans la réalisation du suivi ambulatoire de l'enfant diabétique de type 1, étudier si la télésurveillance peut représenter une solution facilitante envisageable.Matériel et méthode : une étude qualitative par entretien semi dirigé a été réalisée auprès de médecins généralistes de la région Provence-Alpes-Côte d'Azur (PACA). Une analyse inspirée de la théorisation ancrée a été utilisée. Résultats : 13 médecins généralistes ont été interrogés et nous ont permis d'élaborer un modèle théorique. Le DT1 pédiatrique représente la pathologie complexe par excellence. Les problématiques rencontrées par le généraliste sont sources d'une déficience de l'application optimale et de façon performante de certaines de ces compétences. La télésurveillance est globalement bien reçue par le généraliste et semble pouvoir s'intégrer à la pratique de celui-ci sous réserve d'une utilité et d'un cadre adapté. Discussion : la complexité en médecine générale implique des domaines plus larges d'ordre économiques et géopolitiques. Ainsi la prise en charge d'une pathologie telle que le DT1 nécessite des réflexions concernant le système de soins actuel. Alors que le généraliste prêche le travail d'équipe, des enjeux politiques et professionnels entravent sa collaboration avec les nouveaux métiers tel que les infirmiers en pratique avancée (IPA). Toutefois le travail en maison de santé pluridisciplinaire (MSP) semble présenter des avantages conséquents pour la prise en charge ambulatoire des enfants DT1. L'entrée dans le droit commun récente de la télésurveillance, cumulée aux quelques atouts potentiels soulignés par les généralistes, justifient qu'on s'intéresse à son entrée dans nos pratiques.
