Book excerpt: Introduction : les patients âgés institutionnalisés en Établissement d'Hébergement pour Personnes Âgées Dépendantes (EHPAD) et ceux hospitalisés en Soins de Suite et de Réadaptation (SSR) ont un taux de transfert au Service d'Accueil des Urgences (SAU) important en cas d'événement aigu intercurrent. Pourtant le parcours de soins pourrait être plus adapté à leurs besoins. Cette étude décrivait l'état des lieux des trajectoires de soins des patients âgées d'un établissement à la fois EHPAD et SSR lorsque leur état de santé nécessitait une investigation urgente en dehors de sa structure. Matériels et méthodes : cette étude descriptive observationnelle rétrospective s'intéressait aux résidents âgés de plus de 60 ans d'un établissement comprenant 40 lits de SSR et 40 lits en EHPAD dans les Alpes de Haute-Provence sur une période d'un an. Les informations étaient tirées du dossier médical informatisé, rempli par les médecins. Seuls les dossiers comprenant au moins une prise en charge de patient à l'extérieur de la structure pour un épisode aigu étaient analysés. Résultats : 147 mouvements ont été enregistrés devant une pathologie aigüe et concernaient 89 patients de l'établissement. 59,2 % des situations nécessitant une investigation urgente étaient pris en charge en ambulatoire (53,1 % en centre d'imagerie, 6,1% en consultation spécialisée) contre 42,2 % en milieu hospitalier (30,6 % aux SAU, 11,6 % directement en hospitalisation). Les raisons les plus fréquentes de prise en charge à l'extérieur de l'établissement dans notre étude étaient un traumatisme, une pathologie neurologique, une pathologie pulmonaire, une pathologie gastroentérologique. Enfin, plus des deux tiers des patients retournaient dans l'établissement à l'issue de la prise en charge initiale. Conclusion : les médecins d'un établissement médicalisé dont les patients âgés nécessitent une investigation urgente peuvent donc prioriser la prise en charge ambulatoire. Une autre solution pour améliorer la prise en charge de ces patients serait d'améliorer le diagnostic initial, en disposant d'outils diagnostiques sur place (équipe mobile, télémédecine, examens d'imagerie accessibles comme l'échographie).
