Book excerpt: Introduction : Les médecins généralistes se voient attribuer une place centrale dans le dépistage des troubles neurosensoriels de l'enfant. Les recommandations reposent sur la recherche de facteurs de risque, de signes d'appel, et sur l'examen clinique par le biais d'outils. Les médecins généralistes semblent éprouver des difficultés à l'application des recommandations. Pourtant une mallette rassemble des outils avec pour but de faciliter et standardiser le dépistage des troubles neurosensoriels pour les âges clés des 9, 24 et 36 mois. Objectifs : L'objectif principal de notre étude était d'inventorier les usages de la mallette Sensory Baby Test par les médecins généralistes. Méthode : Une étude quantitative descriptive a été réalisée via une enquête diffusée à 402 médecins généralistes utilisateurs de la mallette exerçant dans les régions Bourgogne-Franche-Comté et Grand Est. Les données ont été recueillies de manière rétrospective au moyen d'un questionnaire auto-administré. Résultats : 99 réponses ont été obtenues. Le temps moyen de l'examen était de 27 minutes. La mallette était utilisée par 100 % des médecins à 9 et 24 mois contre 56 % à la visite des 36 mois. Les grilles d'examens systématiques étaient utilisées régulièrement par 42 % des participants. 12 % des médecins avaient bénéficié de la formation recommandée. La principale motivation à l'acquisition de la mallette était l'envie de mieux dépister et les freins majeurs à son utilisation étaient le manque de formation et de pratique. Les utilisateurs la considèrent comme un outil plutôt facile et rapide d'utilisation, bien accepté par les parents et les enfants, mais ils étaient plus divisés sur la reproductibilité. La mallette permettrait aux regards des médecins de notre enquête d'améliorer le dépistage des troubles neurosensoriels et 92% d'entre eux la recommanderaient à un confrère. Discussion : L'utilisation de la mallette Sensory Baby Test pourrait améliorer le dépistage réalisé par les médecins généralistes, à condition de réaliser une formation à son utilisation dans le but d'uniformiser les pratiques. Les formations dispensées actuellement semblent peu adaptées à une diffusion large de l'outil. Si les boitiers sonores et le Test de Lang sont des outils validés, ce n'est pas le cas pour le reste de la mallette. Des tests de validité ou à défaut l'avis des sociétés savantes nous semblent un prérequis indispensable à sa généralisation.

Le repérage des signes d'alerte, l'orientation de ces enfants et la communication avec la famille nécessitent une formation des médecins généralistes dès leur formation initiale. Objectif : l'objectif de cette thèse est de décrire et d'évaluer la mise en place d'une formation à la consultation de dépistage précoce des TND de 0 à 7 ans auprès des internes en médecine générale de Nice. Matériel et méthode : dix internes de tout niveau du diplôme d'études spécialisées (DES) de médecine générale ont assisté à une formation de 3 heures en présence d'une experte en TND. Leur niveau de connaissance avant et après formation a été mesuré à l'aide d'un questionnaire pré et post-test composé de 18 items. Un questionnaire de satisfaction a permis de recueillir l'avis des internes concernant la qualité de la formation. Résultats : la formation a permis une amélioration significative des connaissances concernant les TND. La moyenne du score au questionnaire post-test est supérieure de deux points à celle du questionnaire pré-test avec des items en grande majorité discriminants. Le niveau global de satisfaction des internes était bon avec une note moyenne de 12,3 sur 16 et une formation jugée utile et interactive. Certains participants auraient souhaité plus d'ateliers de mise en pratique des savoir-faire et savoir-être. D'ailleurs, la majorité des internes ne se sentait pas capable de réaliser de manière performante une consultation de dépistage précoce au décours immédiat de la formation. Discussion : bien que l'échantillon soit faible, les résultats du questionnaire post-test, la motivation des participants et la satisfaction globale de cette formation montrent tout l'intérêt d'une telle formation. Le projet de généraliser cette formation à un plus grand nombre d'internes nécessite leur compréhension du rôle central du médecin généraliste dans le repérage des TND.

Ce sont pourtant les principaux acteurs du dépistage de l'enfant. L'objectif principal était d'explorer les écueils de ce dépistage à travers l'étude des pratiques et du ressenti des généralistes sur la faisabilité de ces examens chez l'enfant de 0 à 6 ans. Méthode : Une étude qualitative basée sur des entretiens semi-directifs individuels a été menée auprès de 20 médecins généralistes exerçant en Ille-et-Vilaine, recrutés selon un échantillonnage raisonné. La saturation des données a été obtenue. Une analyse thématique a été réalisée. Résultats : Il en résultait une grande hétérogénéité des pratiques. Les médecins attachaient de l'importance à leur rôle de prévention. Bien qu'ils estimaient ces examens réalisables sous certaines conditions, ils jugeaient leur pratique approximative et subjective. Le manque de formation, de temps et de matériel adapté étaient les principaux obstacles rencontrés. D'autres difficultés concernaient également les médecins les plus sensibilisés à cette pratique : la faible performance et les difficultés d'interprétations des tests ainsi que le problème d'accès à la filière d'aval. Ces freins renforçaient leurs doutes sur la réelle utilité de leur dépistage. Conclusion : Il semble qu'une clarification des recommandations et de certains items du carnet de santé, ainsi qu'un renforcement de la formation initiale soient nécessaires. Au vu de la situation démographique médicale actuelle, le transfert raisonné de compétences vers les paramédicaux spécialisés à des âges clés, et la participation des parents dans ce dépistage sont des solutions à considérer.

The Oral Speech Mechanism Screening Examination (OSMSE) is designed primarily for use by clinical speech-language pathologists. The examination could also serve, however, as a useful tool for oral myofunctional therapists, as well as physical therapists, dentists, and orthodontists interested specifically in speech. It is reliable, relatively easy and quick to administer, and appropriate for children and adults in either diagnostic or therapy settings. The OSMSE is intended to assess those anatomical structures and physiological functions that are most often considered to be potentially related to speech or language disorders. Structural and/or functional judgments included are organized on the OSMSE test form under categories of: lips, tongue, jaw, teeth, hard palate, soft palate, pharynx, breathing, and diadochokinesis. Results of a preliminary study with normal speaking subjects indicated that the OSMSE had satisfactory intra- and inter-examiner reliability after minimal training.
