Book excerpt: Contexte : Le burnout, selon la définition de Maslach, est « un syndrome psychologique ayant trois dimensions : l'épuisement émotionnel, la dépersonnalisation et la réduction du sentiment d'accomplissement personnel ». La prévalence du burnout chez les généralistes est élevée, entre 20 % et 50 % selon les études. Le burnout est source de mal être du médecin et entraîne une diminution de la qualité des soins. Sa prévention est donc un enjeu pour les médecins et la Société. Notre objectif était d'objectiver les moyens de prévention du burnout mis en place ou à mettre en place chez les médecins généralistes libéraux. Méthode : Il s'agit d'une étude qualitative par entretiens semi-dirigés auprès de médecins généralistes installés en libéral dans le Nord-Pas-de-Calais. Les entretiens étaient enregistrés, retranscrits ad integrum en respectant l'anonymat. Les verbatims étaient codés et analysés grâce au logiciel NVivo. Les entretiens ont été arrêtés après saturation des données. Résultats : Neuf entretiens ont été effectués. Parmi les conséquences possibles du burnout des médecins généralistes, celles sur la santé des conjoints ont été évoquées. Les causes évoquées du burnout étaient multiples et variables en fonction du médecin. La prévention du burnout était considérée comme possible. Les médecins généralistes savaient quelles mesures préventives mettre en place sur le plan individuel (savoir dire non, poser des limites, faire le point de temps en temps) et organisationnel (organiser son activité de manière adaptée à soi). La prévention du burnout était difficile à mettre en place au quotidien et devait être régulièrement renouvelée. Une des mesures de prévention mise en avant mais non appliquée était le suivi médical du médecin par un autre médecin. Parmi les moyens de prévention d'avenir étaient évoqués : la mise en place d'une médecine du travail et d'une visite médicale régulière s'appuyant sur des questionnaires ; la mise en place d'un centre pluridisciplinaire indépendant pour les soignants. Conclusion : Nos résultats montrent qu'une meilleure prévention du burnout passerait par un suivi médical régulier du médecin généraliste. Ce suivi pourrait être effectué par un autre médecin ou à l'avenir dans le cadre d'une médecine du travail ou d'un centre pluridisciplinaire indépendant.
