Book excerpt: À une époque d'évolution rapide du système sanitaire, les missions du pharmacien de ville ont évolué pour couvrir de nouveaux aspects de soins. L'impact de ces nouveaux rôles n'est pas encore évalué en soins primaires. En réponse à ce défi, il est important d'étudier cet effet afin de comprendre quelle intervention particulière apporte la plus grande contribution au bien-être des patients. En réalisant une revue systématique de la littérature, nous avons exploré l'impact des différentes interventions pharmaceutiques réalisées par le pharmacien. On a trouvé que la revue médicamenteuse clinique (CMR) est l'intervention la plus étudiée, suivie de l'examen de l'adhésion thérapeutique AR. La CMR a démontré un effet favorable sur différents résultats cliniques, principalement la gestion des problèmes liés aux médicaments et des événements indésirables. Elle a également contribué le plus à la réduction des coûts de santé. L'AR est l'intervention la plus efficace pour améliorer l'observance du patient. La combinaison des deux interventions contribue à augmenter la satisfaction du patient. En considérant ces résultats, il était intéressant de mettre en perspective le rôle du pharmacien d'officine libanais dans l'optimisation de la prise en charge clinique des patients âgés à travers la revue médicamenteuse. Dans ce projet de thèse, nous avons mené des entretiens pharmaceutiques avec les patients âgés libanais prenant quotidiennement plus de 5 médicaments en milieu de soins primaires pour détecter les prescriptions potentiellement inappropriées et optimiser l'usage médicamenteux. On s'est servi de l'outil Bilan de médication élaboré par la société française de pharmacie clinique. Ensuite, on a analysé les prescriptions médicales des sujets inclus en combinant les deux approches d'analyses implicite et explicite. Finalement, les interventions pharmaceutiques ont été rapportées aux médecins, et celles acceptées ont été transmises aux patients. Au total, 850 patients sont inclus dans cette étude. Le nombre moyen de médicaments par ordonnance est de 7. La principale classe de médicaments utilisés est les agents cardiovasculaires. Environ 88% des patients ont au moins une prescription médicamenteuse potentiellement inappropriée. Près de 50,35 % des patients prennent au moins un médicament sans indication. Nous avons recommandé l'arrêt des médicaments pour 76,47 % des problèmes détectés. Seulement 26,57 % de l'ensemble des interventions proposées ont été acceptées. Les patients ont été satisfaits de l'entretien pharmaceutique mené. Comme objectif secondaire, on a aussi évalué les facteurs associés aux prescriptions potentiellement inappropriées. Nos résultats ont montré que certaines caractéristiques sociodémographiques, l'existence de certains syndromes gériatriques, l'observance et l'attitude du patient envers le traitement, sont des prédicteurs positifs de l'existence des PPIs. Finalement, une revue médicamenteuse bien élaborée pourrait être un outil efficace d'identification des patients à haut risque de chute, permettant ainsi l'adoption d'approches préventives ciblées pour améliorer la prise en charge des patients. Ainsi, à la suite des entretiens pharmaceutiques, on a identifié 106 patients qui ont chuté au moins une fois dans les trois mois précédant la revue. Le facteur " chute " a été significativement associée à la présence de syndromes gériatriques (insomnie, somnolence, perte d'appétit et dépendance fonctionnelle), à un nombre plus élevé de conditions médicales, à l'existence d'ostéoporose et à l'utilisation de médicaments qui augmentent le risque de chute notamment les antihypertenseurs anti-adrénergiques et les bêta-bloquants. La revue médicamenteuse s'avère un moyen efficace pour améliorer la qualité des soins offerts aux personnes âgées en soins primaires. L'important étant d'ajouter de la vie aux années et non pas des années à la vie.

L'augmentation de la prévalence des maladies chroniques et l'état polypathologique des sujets âgés sont les principaux facteurs qui expliquent l'augmentation de la consommation médicamenteuse. C'est dans ce contexte que de nouvelles missions du pharmacien d'officine telles que le bilan de médication ont vu le jour afin d'améliorer le bon usage des médicaments chez les patients âgés. L'accès aux antécédents médicaux et aux analyses biologiques des patients qu'implique ce nouveau service permettra aux pharmaciens d'officine de réaliser une analyse pharmaceutique de niveau 3 et de limiter les prescriptions potentiellement inappropriées en collaboration avec le médecin traitant. Pour accompagner cette montée en compétence, le Réseau d'Enseignement et d'Innovation pour la Pharmacie d'Officine (REIPO) structuré en région Occitanie a souhaité engager une campagne de communication et mettre au point avec l'aide de ses partenaires une offre de formation variée. L'objectif de ce travail est de présenter un état des lieux de la mise en œuvre de la campagne de communication et du plan de formation destinés à former les pharmaciens d'officine au bilan de médication. La campagne de communication a permis d'informer plus de 300 pharmaciens de la région Occitanie. Parmi eux, 77 pharmaciens ont signé la charte d'adhésion au REIPO. Le kit de formation élaboré par le REIPO et ses partenaires regroupe des formations à distance et des formations présentielles sur l'optimisation de la prise en charge thérapeutique du sujet âgé. Au sein de ce kit, la Société Française de Pharmacie Clinique a mis au point un blended-learning. Ce dernier est actuellement en cours d'expérimentation, et les premiers retours permettront de faire évoluer les prochaines versions. Les pharmaciens adhérents au REIPO représentent un véritable réservoir de ressources mobilisables, et ils peuvent désormais bénéficier d'une offre de formation variée pour pouvoir être partie prenante dès la rentrée du lancement du projet de déploiement des bilans de médication en région Occitanie (projet BIMED'OC). Ainsi, ils pourront aborder de façon plus sereine la transition qui s'opère pour leur profession.

Le risque iatrogène est donc important, qu'il soit lié au patient, au médicament ou à l'organisation des soins. Le pharmacien est un acteur pour la lutte contre la iatrogénie médicamenteuse, en soins primaires comme à l'hôpital. Ce travail présente un bilan des entretiens d'optimisation thérapeutique effectués par le service pharmacie, pour les patients venant en hospitalisation de jour, au sein de l'hôpital gériatrique de Fourvière (Lyon). Soixante-douze patients venus à l'hôpital de jour ont bénéficié d'un avis pharmaceutique lors d'un entretien, visant à analyser l'ensemble de leurs prescriptions, mais également à évaluer leurs connaissances des traitements et leur observance. Les problèmes retrouvés sur les prescriptions concernaient principalement l'utilisation de médicaments potentiellement inappropriés, le ressenti d'effets indésirables et la non-observance des traitements. Les interventions pharmaceutiques proposées aux médecins, encourageaient une substitution ou un arrêt des thérapeutiques incriminées. Les médicaments les plus retrouvés appartenaient aux classes ATC du système nerveux, du système cardiovasculaire et du système digestif. Dans le courrier de sortie, les avis pharmaceutiques étaient ainsi transmis au médecin traitant. Le recueil de l'acceptabilité des interventions pharmaceutiques par les médecins généralistes n'a cependant pas été possible. Ces entretiens font le parallèle avec les bilans partagés de médication, nouvelle mission du pharmacien d'officine, pour lutter contre la iatrogénie, et ainsi améliorer la prise en charge médicamenteuse des patients âgés.

S'agissant d'entretiens structurés avec le patient, le BPM permet d'optimiser la prise en charge médicamenteuse du patient et a pour objectif, entre autres, de réduire l'iatrogénie, améliorer l'adhésion du patient, réduire le gaspillage médicamenteux et améliorer la collaboration médecin/pharmacien. L'objectif de cette thèse était d'analyser les BPM réalisés dans le cadre d'une expérimentation conduits par des étudiants en 6ème année officine dans les facultés de pharmacie de Lille et Lyon. Cette dernière s'est déroulée de janvier à juin 2018 et a permis d'identifier différentes données sociodémographiques, pharmaceutique et relever certaines difficultés à l'instauration de ces BPM dans le quotidien des officines. De futures expérimentation de ce genre pourraient à terme permettre d'identifier plus de leviers et de freins à la mise en place de cet outil voire d'étudier l'impact pharmacoéconomique du BPM.

Le bilan partagé de médication (BPM) est un entretien structuré mené par le pharmacien chez les personnes âgées polymédiquées. Ses objectifs principaux attendus sont l'optimisation thérapeutique, l'identification des interactions médicamenteuses et la vérification de la bonne observance. Mis en place en 2018, il reste peu utilisé. L'objectif principal de notre travail était d'évaluer la connaissance du BPM chez les médecins généralistes de la région ex-Midi- Pyrénées. L'objectif secondaire était de déterminer les freins et leviers à leur réalisation. Nous avons réalisé une étude observationnelle descriptive au moyen d'un questionnaire auto-administré à destination des médecins généralistes d'Occitanie. Les résultats ont été analysés avec le logiciel Excel et une adaptation en ligne du logiciel R. 48,7% des médecins interrogés connaissaient l'existence du BPM et 26% l'avaient déjà utilisé. L'organisation des BPM n'était pas satisfaisante pour 70% des répondeurs, mais 72% étaient favorables à leur utilisation. Les principaux freins relevés ont été le manque de connaissance de l'outil et le manque de temps. Le BPM est un outil encore peu connu et utilisé. Il met en avant les compétences du pharmacien et développe l'interprofessionnalité dans une démarche d'amélioration de la qualité des soins délivrés au patient. Une meilleure information aux professionnels de santé permettrait au BPM de mieux se déployer bien que le manque de temps à y consacrer reste un frein important.

Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
