Cette dernière, est aujourd’hui mise en place chez les enfants présentant une hémiplégie cérébrale infantile. Elle est basée sur la contention du membre supérieur sain pour favoriser l’utilisation du membre atteint. Cependant, auprès de ce public, elle est difficile à mettre en oeuvre, puisqu’elle est « contraignante ». Il est donc difficile de faire adhérer les enfants à cette thérapie. En pratique, comment l’ergothérapeute peut-il mettre un place un processus de thérapie par la contrainte induite, afin que des enfants présentant une paralysie cérébrale puissent adhérer à ce programme ? L’ergothérapeute, par l’utilisation de la Mesure Canadienne du Rendement Occupationnel, peut fixer des objectifs à atteindre avec l’enfant en fonction des problèmes qu’il rencontre dans ses activités de vie quotidienne. Ces objectifs seront travaillés spécifiquement lors de la thérapie par la contrainte induite, et l’ergothérapeute pourra aider l’enfant à transposer les acquis dans la vie quotidienne, en intégrant une thérapie bi-manuelle en parallèle de la thérapie contrainte. Cela permettra à l’enfant de comprendre l’utilité de cette technique, de pouvoir apprécier l’évolution de l’atteinte des objectifs et ainsi, il pourra mieux adhérer au programme. Cinq ergothérapeutes, mettant en place la thérapie motrice induite par la contrainte auprès d’enfants présentant une hémiplégie cérébrale infantile, ont été rencontrés lors d’entretiens semi-dirigés. Il apparait que la participation de l’enfant à la définition des objectifs favorise son adhésion. Le transfert des acquis dans le quotidien est essentiel : l’enfant peut observer une amélioration de ses capacités dans les activités de vie quotidienne, et cela peut améliorer son adhésion à la thérapie induite par la contrainte. Ce transfert peut se faire par le biais de la thérapie bi-manuelle associée à des objectifs de vie quotidienne. Malgré des facteurs favorisant l’adhésion, un doute est émis sur les capacités cognitives de l’enfant à « adhérer » à une thérapie. La motivation semble être plus adaptée et plus facile à acquérir pour un enfant

Résultats: L'utilisation d'activités ludiques, l'importance du groupe et la valorisation de l'ergothérapeute sont des éléments participant à la motivation de l'enfant. Conclusion: Afin de favoriser la participation de l'enfant à la thérapie par contrainte induite, divers moyens sont à développer par l'ergothérapeute afin de soutenir la motivation de l'enfant.

La Thérapie par Contrainte Induite (TCI), associant contention du membre sain et rééducation structurée du membre atteint, permet d'améliorer les capacités motrices du membre hémiplégique. Elle est le plus souvent réalisée avec un ergothérapeute en structure de soins. Afin de la rendre possible au domicile chez le jeune enfant, nos équipes ont élaboré des scénarios de jeu travaillant la motricité unimanuelle. Ce travail consiste en la rédaction d'un protocole de recherche dont l'objectif principal est d'évaluer l'apport des scénarios de jeu sur l'efficacité de la TCI menée par les parents chez l'enfant hémiplégique de 3 à 5 ans. Méthode : Mise en place d'une étude expérimentale en cas uniques en lignes de base multiples à travers 4 sujets, randomisée, répliquée. Chaque patient participera à 3 phases successives, la 2ème phase étant introduite de façon séquentielle et randomisée. Dans chaque phase, le patient doit porter quotidiennement la contrainte sur une durée libre et être accompagné par un parent pendant 30 minutes de jeux. La phase d'intervention diffère par la nature du jeu imposé qui est le scénario, élaboré à partir des données de l'Assisting Hand Assesment. La « baseline » dure 3 à 7 semaines, les phases « intervention » et « retour à la baseline » 5 semaines chacune. Les parents filment au domicile hebdomadairement l'objectif analysé par la Goal Attainment Scaling, critère de jugement principal, et remplissent quotidiennement le carnet de suivi. Résultats attendus : La supériorité de la TCI avec scénarios par rapport à la TCI avec jeux libres est attendue. L'analyse des résultats aidera à affiner la mise en place de la TCI au domicile. La réalisation de l'étude permettra aussi d'observer la faisabilité de la TCI au domicile avec scénarios de jeux. Conclusion : La TCI est difficile à mettre en place chez l'enfant de 3 à 5 ans de façon intensive en séances de rééducation. C'est une technique de rééducation récente qui consiste à priver l'enfant de son membre supérieur sain grâce une contention acceptable. Le but de cette technique étant de recruter au maximum la plasticité cérébrale de l'enfant et donc par ce biais, permettre une amélioration de la motricité.Objectifs : L'objectif était de démontrer si la CIMT était efficace dans le cadre de l'amélioration de la fonction du membre supérieur, chez une population d'enfants hémi parétiques âgés de moins de 8 ans. Méthode : Nous avons investigué plusieurs bases de données afin de rechercher la littérature nécessaire à la rédaction de cette revue. Les études éligibles à l'inclusion étaient des essais cliniques contrôlés randomisés, l'échantillon traité ne devait pas être âgé de plus de 8 ans. Les enfants devaient être atteints d'une lésion cérébrale entraînant une hémiplégie. Les essais devaient comparer la CIMT aux autres thérapies utilisées actuellement. Nous avons donc choisi de récupérer, de synthétiser et de discuter des résultats des enfants aux tests AHA et QUEST qui représentent la fonction du membre supérieur. Résultats : Les résultats des études incluses ont été analysés de manière qualitative. Ils nous montrent que la thérapie CIMT apparaît plus efficace que la plupart des thérapies utilisées à ce jour à l'exception de la thérapie bimanuelle qui semble la concurrencer chez les enfants âgés de moins de 8 ans. Conclusion : Cette revue a finalement permis de démontrer une tendance à l'amélioration des performances fonctionnelles du membre supérieur chez les jeunes enfants ayant suivi un protocole de CIMT. D'autres études sont encore à mener afin de démontrer l'avantage de débuter une rééducation précoce chez les enfants et permettre dans le même temps d'identifier une mise en place de la rééducation en adéquation avec le jeune âge des patients et leurs besoins.

De ce fait de nouvelles techniques voient le jour afin d'améliorer la prise en charge de cette pathologie, dans la perspective de leur procurer une certaine autonomie. Utilisée chez l'adulte post-AVC, la thérapie par contrainte du mouvement pourrait être prometteuse si elle était davantage développée dans le monde pédiatrique. Objectif : Le but de cette revue de littérature est de déterminer qui, de la thérapie par contrainte induite ou de la thérapie bimanuelle, présente un avantage supérieur dans l'amélioration de la fonction motrice et l'utilisation spontanée du membre supérieur hémiplégique de l'enfant paralysé cérébral. Méthode : Quatre bases de données électroniques et moteurs de recherches ont été investigués (PubMed, Cochrane Library, ClinicalTrial.org, PEDro) d'octobre 2019 à février 2020. Seuls des essais cliniques permettant de répondre à la question clinique ont été inclus. Résultats : 5 études répondant aux critères d'éligibilité ont été analysées. Un total de de 197 enfants, de 8 mois d'âge corrigé à 12 ans ont été inclus dans différents protocoles (de 12 à 96 heures de pratique intensive). Les résultats montrent une amélioration motrice et fonctionnelle dans chacun des deux groupes mais globalement les tailles d'effet obtenues ne favorisent aucune des interventions dans le temps. Conclusion : Il n'est pas possible de mettre en avant une méthode plus qu'une autre puisque les deux présentent des gains similaires. Cependant en fonction des objectifs de traitement, on pourra plus insister sur la TCIM lorsque l'on veut rééduquer spécifiquement le membre atteint, et la TB lorsque l'on veut améliorer la coordination. Ce qui est important est l'intensité mise pour réaliser une tâche.

Fred Busch ponders if it has been fully recognized that some post-Bionions suggest a new, radical paradigm for what is curative in psychoanalysis. After searching for the roots of the analyst’s use of reverie in Bion’s work and questioning whether in this regard Bion was a Bionian, Busch carefully examines the work of some post-Bionians and finds both convincing ways to think about the usefulness and limitations of the analyst’s use of reverie. He explores questions including: From what part of the mind does a reverie emerge? How does its provenance inform its transformative possibilities? Do we over-generalize in conceptualizing what is unrepresented, with the corresponding problem of false positives? Do dreams equal understanding and what about the generalizability of the co-created reverie?
