Des outils destinés à améliorer le repérage et l'accompagnement sont mis à sa disposition. L'addictologie évolue : la prévention des risques et réduction des dommages regroupées sous le terme réduction des risques (RdR) se développe et une nouvelle classification apparaît. L'objectif de cette étude était d'évaluer l'évolution de l'accompagnement des patients. Les objectifs secondaires étaient d'explorer la place du Repérage Précoce et Intervention Brève (RPIB) et l'impact des changements récents en addictologie en pratique. Méthode : Une étude qualitative par entretiens semi-dirigés a été réalisée auprès de 12 médecins généralistes du Nord ayant ou non une compétence en addictologie. Les données ont été analysées par codage à l'aide du logiciel NVivo12®. La saturation des données a été atteinte. Résultats : Le repérage précoce n'était pas correctement intégré. Certains médecins utilisaient l'intervention brève et la trouvaient d'autant plus efficace qu'elle était répétée. Les questionnaires standardisés n'étaient pas utilisés et le changement de classification ne semblait pas avoir d'impact en pratique. Par contre, le changement de paradigme a bien été intégré, tous pratiquaient la réduction des risques et rejetaient les anciennes méthodes. Plusieurs conditions nécessaires à cet accompagnement et des pistes d'améliorations ont été évoquées. Conclusion : Les mesures mises en place pour promouvoir le RPIB ne semblent pas suffisantes, un travail de diffusion reste à faire. Améliorer l'accès à la prévention et l'accompagnement patients présentant un TUA est une des priorités du schéma régional de santé en place depuis 2018. Une évaluation de ce plan est prévue en 2023.

Résultats : cette étude illustre la divergence de perception entre des médecins réticents à aborder ce problème et des patients demandeurs. Les patients sont aussi demandeurs d'information et d'orientation, et expriment un sentiment de manque de formation de leur médecin pouvant entrainer des préjugés. L'opinion des patients sur la place du médecin généraliste rejoint les recommandations de la SFA en ce qui concerne le repérage et l'orientation. Pour la prise en charge, ils sont plus réticents à se faire suivre par leur médecin traitant mais le regrettent paradoxalement. Les patients sont force de propositions en particulier dans l'organisation d'accueil et l'accompagnement de patients souffrant d'un trouble lié à l'usage de l'alcool en soins primaires mais aussi dans le contenu de formation continue. Conclusion : les patients reconnaissent le rôle important de leur médecin traitant dans leur maladie alcoolique. Il serait intéressant de proposer des expérimentations au sein des équipes de soins primaires pour optimiser la prise en charge en médecine générale, et favoriser la coordination entre ces deux niveaux de soins.

L'émancipation du proche dépend majoritairement du sevrage du buveur, tache vécue comme complexe et difficile. Des contraintes multiples impactent le travail du généraliste et les proches en souffrance limitant leur repérage et suivi. Conclusion : les approches des participants discordent avec les recommandations de prise en charge plaçant l'entourage au centre d'un processus où le sevrage des TUA n'est pas impératif. L'éducation des médecins et de la population ainsi qu'un meilleur repérage des ressources semblent utiles.

Les médecins généralistes français, interlocuteurs privilégiés de leurs patients au sujet de leurs consommations d'alcool et acteurs de premier plan du repérage et de la prévention de ces troubles, sous-estiment leur prévalence au sein de leurs patientèles. Quels obstacles rencontrent-ils dans leur repérage ? Leur est-il difficile d'aborder ces troubles ? Quelles-en sont leurs représentations ? Cette étude quantitative a été menée en ligne entre le 16 février et le 16 mars 2016 auprès de 125 médecins généralistes installés ou remplaçants exerçant en libéral dans le département de la Somme. 93,6% des répondants pensaient être les acteurs de santé les mieux placés dans ce repérage. 55,44% n'avaient jamais bénéficié de formation en addictologie. 52,8% ressentaient parfois ou souvent des difficultés à parler d'alcool avec leurs patients. La responsabilité des difficultés rencontrées dans le repérage des troubles de l'usage de l'alcool était très majoritairement imputée aux patients. Le refus des patients de parler d'alcool (91,45%), leurs réticences à la prise en charge (90,67%), le manque de fiabilité de la consommation déclarée (89,74%) et leurs difficultés à reconnaitre leur trouble (88,79%) constituaient des obstacles majeurs au repérage selon les répondants. Ces difficultés sont à mettre en lien avec leurs représentations qui semblent parfois proches des représentations en population générale. 30,97% des répondants percevaient les troubles de l'usage de l'alcool comme un problème de manque de volonté du patient et 17,7% comme un choix de vie. 54,03% des médecins pensaient que ces patients sont souvent plus difficiles que les autres. Les représentations des médecins généralistes, associées à une cible mal définie du repérage, rendent difficile l'abord des consommations d'alcool, avec pour conséquence une recherche trop tardive de ces troubles. La formation à l'addictologie et la clarification du rôle du médecin généraliste seraient des pistes pour améliorer leur repérage.

In 1993, the first version of these guidelines, titled: ‘An outline for the management of alcohol problems: Quality assurance in the treatment of drug dependence project’ was published (Mattick & Jarvis 1993). The Australian Government commissioned an update a decade later (Shand et al. 2003) and a further edition in 2009 to integrate the Guidelines with the Australian Guidelines to Reduce Health Risks from Drinking Alcohol (National Health and Medical Research Council, NHMRC 2009; Haber et al., 2009). The present version of the Guidelines was also commissioned by the Commonwealth of Australia to remain current and integrated with the updated NHMRC consumption guidelines (2020). In order to ensure that guidelines remain relevant, the next set of guidelines should be updated in 2025, consistent with NHMRC recommendation that guidelines be updated every five years. These guidelines aim to provide up-to-date, evidence-based information to clinicians on available treatments for people with alcohol problems and are largely directed towards individual clinicians in practice, such as primary care physicians (general practitioners, nursing staff), specialist medical practitioners, psychologists and other counsellors, and other health professionals. Some chapters highlight service or system level issues that impact on clinicians and their patients. These include recommendations concerning Aboriginal and Torres Strait Islander peoples, culturally and linguistically diverse groups, stigma, and discrimination. Elsewhere, organisation capacity is implied, such as medical resources for withdrawal management where recommendations indicate use of medications. As all forms of treatment will not be readily available or suitable for all populations or settings, these guidelines may require interpretation and adaptation.

The social sciences have been accumulating knowledge since the early fifties as to how we are regulated by others. S. Milgram, S. Ashe, P. Zimbardo and J. Calhoun, detail the vulnerability to being duped and deceived and the difficulty of cooperating when values differ. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
