La survenue de lésions cérébrales par accident vasculaire ou traumatisme crânien peut induire une diminution des capacités de conduite. Le but de ce travail est de déterminer l'évolution des capacités de conduite, suite à l'évaluation sur simulateur. Il fait un résumé des évaluations sur simulateur de conduite sur cinq années et le compare aux démarches prises par les patients lors de leur reprise de conduite. Méthode : Dans cette étude rétrospective monocentrique, 287 patients étaient inclus, atteints d'accident vasculaire cérébral, de traumatisme crânien, ou toutes autres pathologies cérébrales acquises, non évolutive. Les patients étaient recrutés de 2011 à 2015, en date de leur passage sur simulateur de conduite automobile à l'hôpital Swynghedauw du CHRU de Lille. Résultats : Les évaluations sur simulateur ont permis de conclure des capacités compatibles avec la reprise de conduite pour 140 patients (48.8%). Dans 46 cas (16%), il était conseillé de ne pas reprendre la conduite car les capacités ne semblaient pas compatibles. Un approfondissement des tests en auto-école a été nécessaire pour 61 patients (21.3%). Un réentrainement sur simulateur était proposé à 11 patients (3.8%). Un aménagement du poste de conduite était recommandé pour 71 patients (24.7%). Définie par l'OMS comme une fonction humaine faisant partie intégrante de la vie quotidienne, la conduite demeure une activité complexe. En effet, elle requiert certaines fonctions cognitives développées dans le modèle théorique de Michon de 1985. Le but de ce travail de recherche est d'étudier l'acte d'évaluation de cette activité, pratiqué par les ergothérapeutes. Pour cela, nous avons interrogé lors d'entretiens semi-directifs, quatre ergothérapeutes expérimentés dans ce domaine. Il en ressort que bien que les modalités diffèrent, chacun utilise dans un premier temps des bilans neuropsychologiques puis une mise en situation écologique pratiquée grâce à une auto-école spécialisée. La complémentarité existante entre ces deux modes d'évaluation permet alors d'émettre un avis pertinent pouvant accompagner le médecin agréé Préfecture dans sa décision.

Cependant, aucune grille d' évaluation standardisée sur route n' est actuellement validée, seule la TRIP a fait l'objet d' une validation, exclusivement dans la population gériatrique. Objectif : l'objectif de ce travail est de créer puis valider une nouvelle grille d' évaluation, à partir de la TRIP et des recommandations de la HAS, permettant d' évaluer l' aptitude à la conduite après un AVC. Méthode : une étude préliminaire, dite prétest, a été réalisée à partir des deux grilles (TRIP et HAS) avant et après élaboration d' une nouvelle grille issue des recommandations de la HAS et de l' avis d' experts. L'étude ROADTRIP étudie les propriétés métrologiques de cette nouvelle grille à partir des évaluations des patients lors de 1, 2 ou 3 tests sur route accompagnés d' un moniteur d'auto-école, par un ergothérapeute et un neuropsychologue puis un ergothérapeute seul. Résultats : vingt et un (21) patients au total ont été évalués pour le prétest, dont 11 à partir des grilles TRIP et HAS, la première se montrant simple d' utilisation, avec des items pertinents mais ne couvrant pas l'ensemble des axes attendus, et la deuxième se montrant difficilement utilisable en pratique (72 % des items étant non évaluables). La nouvelle grille créée a ensuite été évaluée lors du prétest pour 10 patients, en complément de la TRIP. Au total, 13 patients sur 100 attendus ont été inclus dans l'étude ROADTRIP au moment de la rédaction de ce travail. L' analyse qualitative préliminaire met en évidence une acceptabilité correcte, une reproductibilité inter-juges de 82 % et du retest de 73 %. Conclusion : la faible puissance actuelle de l'étude ne permet pas de conclure à la validité de la grille, cependant l' acceptabilité et la reproductibilité semblent acceptables.
