Book excerpt: Les étudiants en médecine de fin de 6e année estiment manquer de compétence au finir de leur externat. Les stages hospitaliers doivent améliorer ce déficit en pourvoyant un enseignement contextualisé. Pour ce faire la mise en place d'objectifs propres à notre service de médecine interne à Bordeaux est une première étape vers l'amélioration de nos capacités pédagogiques. METHODES : Nous avons pris comme référentiel les finalités et objectifs du bulletin officiel de 2007 relatif au deuxième cycle des études médicales. Parmi les items de l'examen classant national (ECN) nous avons sélectionné ceux paraissant les plus pertinents par rapport au terrain d'enseignement que constitue le stage. Nous avons aussi défini des objectifs en rapport avec des bases de séméiologie, et des gestes techniques réalisés au sein du service. Concernant les modalités de l'enseignement, il s'agissait d'un accompagnement au lit du malade. RESULTATS : Un document définissant les objectifs de stage a été mis en place, composé notamment de 38 items de l'ECN, des éléments relatifs à la séméiologie, et à la réalisation de 7 gestes techniques. Ce document est systématiquement remis aux étudiants à leur arrivée en stage pour mieux les aider à s'investir et acquérir des connaissances pratiques. L'analyse d'un questionnaire d'évaluation rempli par 25 étudiants montre que les choix réalisés pour constituer ce document sont adaptés quantitativement et qualitativement. Les externes expriment une bonne satisfaction envers cet outil pédagogique mais pointent du doigt l'insuffisance des gestes techniques vus ou réalisés. L'ensemble de ces éléments nous amène à poursuivre l'utilisation de ce document, et entamer une réflexion sur les modalités de l'enseignement afin de parfaire le nombre d'objectifs atteints par tous en fin de stage. A ce titre un outil de planification a été élaboré. CONCLUSION : Notre travail a permis de mettre en place un outil pratique définissant les objectifs de stage dans notre service de médecine interne. Les étudiants sont satisfaits de ce nouvel outil qui leur permet de mieux cerner l'enseignement qui leur sera donné.

Il se base sur une méthode d'apprentissage par compétences, issue des théories constructivistes. Le Stage Ambulatoire en Soins Primaires en Autonomie Supervisée (SASPAS) est l'occasion pour l'interne de renforcer ses compétences professionnelles, par la confrontation en autonomie aux grandes familles de situations cliniques représentatives de la discipline.Objectif: Il s'agissait d'élaborer un livret d'exercices pédagogiques pratiques à destination des internes en SASPAS. Cet outil formatif se voulait exploitable par l'interne en situation de consultation en soins premiers. Le but était de faciliter l'acquisition contextualisée des six compétences génériques de la profession .définies par le Collège National des Généralistes Enseignants.Méthode et résultats: Nous avens consulté par mail les maîtres de stage et les internes de médecine générale rattachés à la faculté de: Dijon, pour recueillir leurs suggestions concernant le contenu des exercices pédagogiques. Vingt-trois exercices ont été ensuite élaborés par une interne de médecine générale. Leur contenu a été évalué par une équipe d'enseignants. Un argumentaire pédagogique a· été créé, décrivant pour chaque exercice: les moments de consultation et les familles de consultations cliniques ciblés, les compétences abordées, le niveau de compétence de l'exercice assorti de descripteurs de niveau, le type de savoir mobilisé, ainsi qu'un exemple ·de résultat d'exercice. Sept exercices issus de travaux antérieurs d'une équipe dijonnaise ont été intégrés au livret. Chaque exercice tentait d'orienter la réflexivité de l'interne en situation réelle sur .une ou plusieurs compétences de médecine générale. Leur bilan devait être ensuite réalisé avec le maître de stage lors de la supervision indirecte.Conclusion : Notre travail a permis la création d'un livret de trente exercices pédagogiques exploitables par les internes en SASPAS. La diffusion de ce livret aux maîtres de stage devrait permettre de fournir un support pédagogique contextualisé ayant pour finalité une supervision indirecte enrichie.
