This was the first International Conference held under the auspices of the International Society for Behçet’s Disease which was founded in 2000 in Seoul. First, I congratulate our colleagues in Berlin, led by Professor Christos Zouboulis of the Department of Dermatology at the Free University of Berlin, for having organised a most successful conference and for having compiled these proceedings so rapidly. It will be realised immediately on scanning the contents of this book that the conference was truly international with 210 participants from 26 countries, as Professor Zouboulis has noted in his preface. These included basic scientists, epidemiologists, pathologists, clinicians and, importantly, representatives from patient organisations. The latter held their own conference alongside the scientific-medical conference to mutual benefit. The combined session of patients and doctors (abstracts on pp 601 – 626) gave the opportunity for an exchange of information and fruitful discussion. The wide ranging scope of the communications is evident from the index and it was most encouraging to see their origin – from all parts of the world, from senior and junior colleagues and, from many different disciplines. Many communications may be regarded as preliminary reports of research in progress and we look forward to seeing the definitive publications in appropriate journals in due course.

Book excerpt: Contexte : Le médecin généraliste est un acteur essentiel de la lutte contre l'émergence des entérobactéries productrices de béta-lactamase à spectre étendu (EBLSE). Les recommandations de la Société de pathologie infectieuse de langue française (SPILF) 2014 encadrent pour la première fois la prise en charge des infections urinaires à EBLSE. Objectif : Le but de cette étude était de décrire la prise en charge des infections urinaires à EBLSE en médecine générale, et d'évaluer l'intérêt des médecins pour cette problématique. Méthode : Une étude épidémiologique observationnelle descriptive a été menée entre le 20 mars et le 30 mai 2015 auprès de 350 médecins généralistes tirés au sort en France métropolitaine. Un questionnaire basé sur un cas clinique a été envoyé à chaque médecin. Quatre cas cliniques différents ont été créés : cystite aiguë simple, cystite aiguë à risque de complication, pyélonéphrite aiguë simple et infection urinaire masculine aiguë fébrile. Le critère de jugement principal correspondait à la prescription d'une antibiothérapie de première intention, sans notion de durée (antibiothérapie de première intention), et pendant la bonne durée (adéquation totale). Le référentiel était les recommandations de la SPILF 2014. Résultats : Nous avons recueilli les réponses de 150 médecins. Parmi eux, 56,4% avaient déjà pris en charge un patient infecté à EBLSE et 69% se sentaient concernés par l'émergence de ces infections. Les critères d'adéquation totale et d'antibiothérapie de première intention concernaient respectivement 20,1% (IC95 [14,1-27,8]) et 36,8% (IC95 [29-45,3]) des médecins. Il existait une association statistiquement significative entre la lecture des recommandations de la SPILF 2014 et l'adéquation totale (p=0,0126) ou l'antibiothérapie de première intention (p=0,0362). La durée de traitement était trop longue dans 48,6% des cas, allongement dû à l'EBLSE dans moins d'un cas sur deux. Au total, 60% des prescriptions de fluoroquinolones étaient évitables. La majorité des médecins était favorable à la diffusion d'une aide à la prise en charge des infections à EBLSE (« kit BLSE », réfèrent en infectiologie). Conclusion : La sensibilisation des médecins à la problématique des infections à EBLSE doit être intensifiée. Plus encore, la prise en charge des patients atteints d'une infection urinaire à germe multi-sensible et la lutte contre le mésusage constituent une priorité absolue. L'échange entre professionnels de santé doit être renforcé.
