Elle se manifeste par des poussées de durée variable et des périodes d'accalmie. C'est une maladie auto-immune caractérisée par la fabrication d'auto-anticorps dirigés contre la membrane synoviale des articulations. Sans traitement, la maladie atteint progressivement de nouvelles articulations et entraîne la déformation ou la destruction progressive des articulations touchées. En France, la polyarthrite rhumatoïde touche 0,3 à 0,8 % de la population adulte, soit environ 200 000 personnes. Elle est deux à trois fois plus fréquente chez les femmes que chez les hommes. Elle apparaît le plus souvent entre 40 et 60 ans, autour de la ménopause. Actuellement, il n'existe pas de remède contre la polyarthrite rhumatoïde. Les traitements visent donc à soulager la douleur et la raideur et à améliorer la capacité de la personne à se déplacer et à vivre au quotidien. Les maladies parodontales sont plus fréquentes chez les sujets atteints de polyarthrite rhumatoïde. L'inflammation et les déformations articulaires rendent plus difficiles le contrôle de plaque dentaire. Les troubles de l'articulation temporo-mandibulaire touchent fréquemment les patients atteints de polyarthrite rhumatoïde, enfin le syndrome de Gougerot-Sjögren accompagne fréquemment cette maladie. La première partie de ce travail consistera en des rappels basés sur les données récentes de la littérature concernant cette pathologie : définition, physiopathologie, épidémiologie, diagnostic, différentes stratégies thérapeutiques. Dans la seconde partie de ce travail, nous aborderons les différentes manifestations buccales directement ou indirectement engendrées par la polyarthrite rhumatoïde ainsi que les différentes conduites à tenir lorsqu'un patient atteint de cette maladie se présente au cabinet dentaire.

Ceci a été recensé dans une étude de 46 personnes malades hospitalisées dans les services de rhumatologie de l'hôpital Lariboisière. Il en résulte de réels problèmes parodontaux, d'ATM, de stomatites et candidoses, mais il ne s'agit pas d'une population au risque carieux accru. Ces observations ont permis l'élaboration de recommandations pratiques destinées aux malades, pour les aider dans leur quotidien, et aux professionnels de santé, dentistes et rhumatologues, pour améliorer la prise en charge bucco-dentaire de ces personnes atteintes de Polyarthrite Rhumatoïde.
