Le nombre d'échographies obstétricales au cours dd'une grossesse physiologique ne cesse dd'augmenter. Cet examen est devenu incontournable pendant la grossesse. Pourtant la femme enceinte semble peu informée et se dit déçue après sa réalisation. Objectif. Explorer les connaissances et les attentes de l'échographie obstétricale de dépistage chez la femme en début de grossesse. Méthode. Étude qualitati ve par entretiens individuels semi dirigés menés entre janvier 2019 et mars 2020 aupr è s dd'un échantillon varié de 9 femmes enceintes en début de grossesse. Résultats. Initialement perdue dans les démarches, la femme enceinte a le réflexe de prendre rendez vous sans prescription pour une échographie obstétricale. Le lien entre grossesse et échographie est un lien d'évidence. En passant à côté de la prescription médicale, elle ne reçoit aucune information. Elle ne se renseigne pas sur la réalisation de ll'exa men ni sur les effets secondaires possibles la femme enceinte fait confiance au corps mé dical. Elle idéalise le temps de l'échographie au travers des images vues dans les médias. L'échographie peut être ressentie comme une expérience traumatisante. Pour se sentir mieux accompagnée, la femme enceinte évoque le besoin dd'un support écrit reprenant les étapes et les examens en lien avec la grossesse ainsi que les informations importantes. Discussion. Le processus de parentalité ne débute plus avec les premi ers mouvements foetaux ressentis mais avec ll'image créée par l'échographie obstétricale. La femme devient responsable de l'être qui grandit en elle. Cette responsabilité semble à l'origine d'une forte adhésion au programme de dépistage des anomalies foetales par l'échographie. Si la femme enceinte ne sait pas vers qui se tourner en début de grossesse, de nombreuses opportunités s'offrent aux soignants pour anticiper cette étape et informer la femme lors de sa vie fertile.

Book excerpt: La consultation de diagnostic prénatal représente un moment singulier de la grossesse pouvant perturber son cours psychique. Notre étude ne s’intéresse pas au diagnostic lui-même mais à son contexte, à sa forme, plus particulièrement à la valeur de l’incertitude qui en découle et son action sur le développement de la parentalité et du lien mère-enfant. Nous définissons d’abord le cadre du diagnostic prénatal, puis reprenons la littérature portant sur l’évolution du psychisme maternel au cours de la grossesse et sur les aspects psychiques de l’échographie obstétricale. Nous présentons ensuite notre protocole de recherche et nos résultats sous la forme combinée d’une étude statistique sur un panel de 19 patientes consultant pour la première fois dans un service de diagnostic anténatal, et de l’analyse longitudinale de quatre cas cliniques pendant la grossesse et après l’accouchement. Notre étude conclut d’une part à l’existence d’une symbolique forte même dans les gestes les plus anodins d’une échographie, notamment via l’émergence d’un « ventre passoire » au cours d’examens prénataux faisant disparaître l’opacité du ventre maternel et révélant la « transparence psychique » relative à l’état de grossesse ; d’autre part, elle affirme la prépondérance de la parole sur le regard comme moteur du développement psychique de la femme enceinte ; enfin, sans que soit remise en cause l’exactitude du diagnostic, elle décèle dans la relation avec l’échographiste une part d’incertitude inhérente au « malentendu fondamental » qu’il ne faut pas s’attacher à gommer tant elle est propice à l’attachement maternel et à une relation saine entre une mère et son enfant.
