Cependant, les problèmes de fractures d'implant, de pilier et les phénomènes de dévissage de vis de pilier implantaire, qui sont d'incidence non négligeable, nous ont amenés à réaliser cette étude sur l'élément essentiel des systèmes implantaires: la jonction entre l'implant et le pilier appelée connexion implantaire. En effet, la première partie de la thèse met en évidence la résistance mécanique des connexions implantaires ou par quel moyen cette connexion va prendre en charge les forces masticatoires en les transférant de manière optimale à l'implant. Pour ce faire, nous avons comparé les connexions dites passives aux connexions dites actives. D'autre part, il nous a semblé intéressant de traiter aussi des critères cliniques rentrant en compte dans le choix d'une connexion par rapport à une autre: ainsi, après avoir établi une classification non exhaustive des systèmes de connexions implantaires, nous avons abordé des considérations ergonomiques, esthétiques et parodontales. Nous avons conclu par un tableau de synthèse comparant les différents types de connexion des systèmes implantaires entre eux, nous amenant à la conclusion qu'il n'existe pas de connexion idéale réunissant tous les critères cités; il s'avère toutefois conseillé d'utiliser des connexions de sociétés implantaires fiables et solides financièrement dans la mesure où par la suite nous devons effectuer la maintenance.
