Les mesures de base, les Précautions Standard (PS), à appliquer au cabinet de ville peuvent être complétées par des Précautions Complémentaires dans des situations à haut risque de transmission croisée telles que les Bactéries Hautement Résistantes aux antibiotiques émergentes (BHRe) ou la tuberculose pulmonaire. Cette étude vise à évaluer le niveau d'application des précautions d'hygiène adaptées lors de la prise en charge d'un patient à risque infectieux spécifique par les médecins généralistes. Méthodes : l'étude a été réalisée entre le 21 juin et le 15 septembre 2017 par questionnaire - envoyé par courrier - auprès des 55 médecins généralistes libéraux du Perche, en Eure-et-Loir et dans l'Orne. Les 54 items recueillis ont été élaborés en quatre catégories selon les données de la littérature et les recommandations: caractéristiques personnelles, connaissances concernant le risque infectieux en général, les Bactéries MultiRésistantes (BMR)/BHRe, le risque respiratoire, ressources matérielles au cabinet et organisation de la salle d'attente. Résultats : le taux de réponse a été de 60 % (33/55) avec une majorité d'hommes (76 %) et un âge moyen des médecins de 55,5 ans. Un quart des 33 médecins répond avoir reçu une formation sur la gestion du risque infectieux. Deux-tiers des médecins effectuent une hygiène des mains entre chaque patient mais seulement 55 % utilisent les Produits HydroAlcooliques (PHA). Trente médecins connaissent les BMR contre cinq les BHRe. Un tiers porte des gants par excès pour examiner un patient porteur de BMR. Moins d'un quart des médecins portent un masque chirurgical en prévention de la diffusion des infections respiratoires. Pourtant les praticiens ont à disposition les PHA dans 91% des cas, les gants dans 97% et les masques dans 88%. Conclusion : les ressources sont bien à disposition dans les cabinets médicaux pour une bonne application des précautions d'hygiène. Les médecins de cette région manquent d'information et de formation concernant les BMR/BHRe et la prévention des infections respiratoires. En conséquence, le niveau d'application des mesures d'hygiène adéquates est à améliorer incluant l'application des PS. Les résultats de ce travail nous ont invités à créer et à distribuer une plaquette d'information sur les risques spécifiques BMR/BHRe ou infectieux respiratoires pour guider les médecins généralistes dans la prévention de ces risques.

Les thèmes qui apparaissent maîtrisés sont l'équipement du cabinet médical en point d'eau et en matériel, les vaccinations des professionnels de santé et la gestion des déchets. Les thèmes qui restent à acquérir sont : le respect des précautions standard et notamment l'hygiène des mains par l'utilisation de produits hydro-alcooliques, la désinfection et la stérilisation du matériel médical, la gestion des accidents d'exposition aux liquides biologiques et l'adéquation de l'usage des antiseptiques en fonction des situations de soins. Les principaux freins à l'adoption des recommandations sont le manque de formation initiale et continue, la perception d'un risque infectieux faible ou modéré, le manque de temps, le coût et l'investissement pour se conformer aux recommandations.

Objectif : Identifier les caractéristiques des patients associées à l'hygiène des mains des médecins généralistes lors des visites à domicile Méthode : Etude épidémiologique descriptive transversale quantitative par questionnaires anonymes envoyés à 1162 médecins généralistes du Val-de-Marne en 2016Résultats : Dans notre population d'étude de 87 médecins, l'hygiène des mains des médecins avant et après examen a été significativement associée aux patients infectés [avant examen, p=0,60 vs patient sans comorbidité : p=0,55 ; p-value=0,0041 (Xi2, sans correction de Yates) / après examen, p=0,86 vs p=0,64 ; p-value=0,00084] et aux patients ayant une porte d'entrée infectieuse [avant examen, p=0,83 vs p=0,55 ; p-value=0,000084 / après examen, p=0,81 vs p=0,64 ; p-value=0,018]. L'hygiène médicale des mains avant examen a été significativement associée aux patients immunodéprimés [p=0,84 vs p=0,55 ; p-value=0,000038] et aux nourrissons [p=0,83 vs p=0,55 ; p-value=0,000084].Conclusion : Les recommandations pourraient être adaptées à la médecine générale en ciblant les patients à risque infectieux élevé lors des visites à domicile.

Book excerpt: "Hypoxaemia is a major contributor to child deaths that occur worldwide each year; for a child with pneumonia hypoxaemia increases the risk of death by up to 5 times. Despite its importance in virtually all types of acute severe illness, hypoxaemia is often not well recognized or well managed, more so in settings where resources are limited. Oxygen therapy remains an inaccessible luxury for a large proportion of severely ill children admitted to hospitals in developing countries. This is particularly true for patients in small district hospitals, where, even if some facility for delivering oxygen is available, supplies are often unreliable and the benefits of treatment may be diminished by poorly maintained, inappropriate equipment or poorly trained staff with inadequate guidelines. Increasing awareness of these problems is likely to have considerable clinical and public health benefits in the care of severely ill children. Health workers should be able to know the clinical signs that suggest the presence of hypoxaemia and have more reliable means of detection of hypoxeamia. This can be achieved through more widespread use of pulse oximetry, which is a non-invasive measure of arterial oxygen saturation. At the same time oxygen therapy must be more widely available; in many remote settings, this can be achieved by use of oxygen concentrators, which can run on regular or alternative sources of power. Having effective systems for the detection and management of hypoxaemia are vital in reducing mortality from pneumonia and other severe acute illnesses. Oxygen therapy is essential to counter hypoxaemia and many times is the difference between life and death. This manual focuses on the availability and clinical use of oxygen therapy in children in health facilities by providing the practical aspects for health workers, biomedical engineers, and administrators. It addresses the need for appropriate detection of hypoxaemia, use of pulse oximetry, clinical use of oxygen and delivery systems and monitoring of patients on oxygen therapy.
