Book excerpt: INTRODUCTION : La douleur induite par les soins reste insuffisamment prise en compte dans la pratique clinique quotidienne. Cette étude vise à effectuer un état des lieux de la prise en charge des douleurs induites par les soins chez les patients hospitalisés au sein du CGFL. METHODE : Une étude rétrospective descriptive a été menée à travers 200 entretiens, standardisés par un questionnaire, réalisés entre le 7 novembre 2023 et le 1er février 2024, auprès des patients hospitalisés du CGFL. RESULTATS : Les patients rapportent qu'une évaluation de leur douleur par le soignant a été faite dans 92,5% des cas avant le soin. Un traitement préventif antalgique a été proposé par le soignant dans 79,5% des cas. Ces traitements ont été décrit comme efficaces par les patients dans 87,4% des cas. Ils rapportent que le soin leur a été expliqué dans 95% des cas. Les patients ont répondu qu'ils étaient satisfaits de la prise en charge des douleurs induites par les soins dans 95% des cas. On notera que le soin potentiellement douloureux le plus rapporté par les patients est la pose de perfusion IV. Les formes de traitements inhalées et non-médicamenteuses sont les moins souvent proposées. CONCLUSION : Cette enquête a permis d'établir une vue d'ensemble de la prise en charge des douleurs induites auprès des patients du CGFL. La fréquence de proposition du traitement préventif par les soignants peut encore être améliorée, non réalisée dans 20,5% des cas. Un regard plus précis concernant, chaque type de soin individuellement, l'utilisation des échelles d'évaluation de la douleur et les thérapies non-médicamenteuses et inhalées, pourraient faire l'objet de travaux de recherches ultérieures.

Elle peut être ressentie par le patient lors d'un soin, on l'appelle alors douleur induite. Ces douleurs sont prévisibles et des moyens médicamenteux ou non permettent de les prévenir. Mais ces moyens n'ont pas toujours existé, la douleur étant longtemps considérée comme le prix à payer pour la guérison. Bien que la prise en compte de cette douleur s'améliore, elle demeure encore probablement sous-estimée. Sont mis en cause en particulier le peu de protocoles antalgiques dédiés à ce type de douleur et leur manque de normalisation, et un personnel soignant insuffisamment sensibilisé. Un défaut de prise en charge peut générer chez le patient, mais aussi chez le soignant, appréhension, stress, refus de soin, perte de confiance. Des échelles d'auto et hétéro évaluation de la douleur sont utilisées avant, pendant et après les soins afin d'adapter au mieux la prise en charge, et l'utilisation d'une échelle de référence est primordiale. La prise en charge médicamenteuse est largement utilisée avec des antalgiques de palier I, II et III, l'anesthésie locale et le MEOPA (Mélange Equimolaire Oxygène Protoxyde d'Azote). De nombreuses méthodes non médicamenteuses se développent également, comme l'hypnose. Une étude déclarative a été réalisée auprès des soignants des unités de soin de 4 établissements de gériatrie du CHU de Lyon (Croix Rousse, Pierre Garraud, Antoine Charial et Dugoujon) grâce à un questionnaire. Les résultats montrent que le geste le plus fréquemment et intensément douloureux est le prélèvement pour gaz de sang. Tous les gestes ont été considérés comme fréquemment ou très fréquemment douloureux, toutes professions confondues mais concernant l'intensité de la douleur nous retrouvons une différence de perception entre les professions. Pour la prévention de la douleur induite par les soins de plaies, une prémédication est largement mise en place, tous établissements, professions et unités de soins confondues, grâce à des traitements non morphiniques, morphiniques et le MEOPA. Pour les prélèvements veineux, la prévention est faible. Il y a une disparité entre les établissements questionnés avec un établissement dont les soignants répondent pour la plupart du temps ne jamais mettre en place de prémédication. De plus, les réponses sont différentes entre les professions avec des médecins disant pour la majorité mettre en place une prémédication fréquemment contre des infirmiers n'en mettant jamais. Même si cette étude ne nous permet pas des analyses statistiques très poussées en raison d'un nombre limité de réponses, elle a permis de dégager plusieurs perspectives de travail sur les méthodes de prévention utilisées et leur efficacité. Cette étude souligne également la difficulté liée à l'estimation externe de la douleur, et l'influence de facteurs comme la profession de l'observateur sur l'évaluation de l'intensité de la douleur induite par les soins.

Même si les soignants se disent sensibilisés au questionnement de la douleur, le dépistage et la prévention de la douleur induite par ces soins ne sont pas systématiques. Leur savoir-faire repose essentiellement sur leurs expériences. Leurs connaissances, souvent jugées insuffisantes, ne satisfont pas les soignants qui souhaitent se former davantage. La complexité de la prise en charge de la douleur induite par les soins justifierait une approche globale avec mise en commun des compétences du médecin et de l'infirmier. Cependant les soignants restent dans des rôles cloisonnés, avec des missions distinctes ne permettant pas de développer leur complémentarité. Ceci amène à une réflexion sur l'amélioration de l'organisation des soins avec la nécessité que médecins et infirmiers travaillent plus ensemble, dans un esprit de collégialité avec le patient. Il apparaît également nécessaire que les soignants se forment et soient convaincus de l'intérêt de dépister et de prévenir la douleur induite lors de ces soins.
