Book excerpt: La Haute Autorité de Santé définit le syndrome douloureux chronique comme un syndrome multidimensionnel, dont la douleur exprimée, quelles que soient sa topographie et son intensité, persiste ou est récurrente au-delà de ce qui est habituel pour la cause initiale présumée, répond insuffisamment au traitement, ou entraîne une détérioration significative et progressive des capaciés fonctionnelles et relationnelles du patient. L'étiopathogénie de ce syndrome est multifactorielle, ce dernier résultant d'évènements somatiques, psychologiques, environnementaux et sociaux, venant en favoriser la chronicisation. Nous avons réfléchi à la complexité du phénomène douloureux à travers une revue de la littérature et la présentation clinique d'un patient rencontré en consultation de psychiatrie de liaison au Centre d'Evaluation et de Traitement de la Douleur du CHU de Besançon. La douleur chronique peut jouer le rôle d'un véritable mécanisme de défense, et contribuer à protéger et à maintenir l'équilibre psychique du sujet. Ce mécanisme est néanmoins toujours pathologique, car la douleur envahit le patient et retentit dans son corps, son psychisme et dans tout son système relationnel. Dans certains cas, la douleur chronique pourrait venir symboliser un processus de deuil. Elle contribuerait à bloquer l'élaboration psychique nécessaire lors de tout travail de deuil, la représentation psychique de la perte entravée par la douleur incarnée dans le corps. Mais elle semblerait également protéger le sujet d'une décompensation mélancolique et porterait donc un potentiel de transformation pouvant relancer les processus d'élaboration psychique liés à la perte. La douleur chronique nécessite une prise en charge multidisciplinaire au sein de laquelle le psychiatre a une place essentielle.

The act of the other's death is essentially a decease, a break affecting the identity. It forces man to consider the fundamental intersubjectivity inscribed in his temporality. Viewed in this way, death does not look merely like the term of life coming to an end. Nor is it a passage to `somewhere beyond'. Rather, it lies at the core of the act of relationship. In its search in the space between sense and non-sense, this phenomenology of death reveals the fundamentally relational dimension of the humane and sketches the main features of this paradoxical `intersubjectivity': the position of third party that is taken by man, the calling of son that he has been selected for and - midway between passivity (Levinas) and possibility (Heidegger) - the condition of `liability' to which he is dedicated and of which he is also worthy.

This volume draws together examples from throughout the Francophone world, including countries such as Algeria, Morocco, Lebanon, Rwanda, Cameroon, Congo, Côte d’Ivoire, Haiti, New Caledonia, Quebec and France, and across genres such as autobiography, poetry, theater, film, fiction and visual art to provide a cohesive analysis of the representation of trauma. In addition to the survivors’ expression of trauma, the witnesses and receivers are also taken into account. By gathering studies that explore diverse bodily and psychological traumas through tropes such as repetition, silence and working-through, it tackles ethical responsibility and interrogates how expressive forms evoke a terrible reality through the use of imagination. The aim of this volume is not to question if suffering is representable, but rather to examine to what extent art surpasses its own limitations and goes straight to its essence. The Unspeakable hopes to provide models for the cultural translation of trauma, because, when represented and released from silence and isolation, trauma can give way to the arduous process of healing.
