En effet la pose d'un implant nécessite une quantité d'os suffisante pour assurer la rétention primaire de l'implant et par la suite son ostéo-intégration.De nombreuses techniques chirurgicales ont été mises au point dans le but d'aménager ce volume osseux. On peut citer la greffe osseuse, le soulevé de sinus, la régénération osseuse guidée, ou bien encore la distraction alvéolaire. Cette dernière consiste à séparer une pièce osseuse en deux et à espacer graduellement les deux fragments dans le but d'obtenir, après cicatrisation, un os plus grand. L'objectif de ce travail est de mettre en évidence les indications et les limites de la distraction alvéolaire. Après avoir définis les spécificités de la cicatrisation par distraction, nous nous intéresserons à la technique opératoire de la distraction osseuse alvéolaire. Nous comparerons ensuite la distraction aux autres techniques d'augmentation du volume osseux. Nous conclurons en déterminant la place actuelle de cette technique en implantologie orale.

Pour que celle-ci soit possible, la hauteur osseuse alvéolaire doit être suffisante pour permettre la pose d'implants dans de bonnes conditions. C'est le but des techniques de chirurgie pré-implantaire avec, parmi elles, la distraction alvéolaire. PATIENTS ET METHODE : Il s'agissait d'une étude rétrospective regroupant 33 patients ayant bénéficié d'une distraction alvéolaire par un système intra-oral et extra-osseux, sur le secteur mandibulaire, entre 1999 et 2007, pour un total de 47 sites de distraction. L'étude clinico-radiologique a permis de mesurer le gain osseux, les complications, et le taux de succès implantaire final. RESULTATS : 91 % des patients ont bénéficié de la mise en place de 116 implants sur un total de 44 sites mandibulaires, avec un taux de succès implantaire en fin d'étude de 95,7 %. CONCLUSION : La technique de distraction alvéolaire est fiable et permet la réhabilitation orale implanto-portée finale dans la plupart des cas. Les complications sont acceptables et peuvent être diminuées par une bonne connaissance de la technique. La distraction alvéolaire a toute sa place dans le domaine de la chirurgie préimplantaire, à condition d'en respecter les indications.

Jusqu’à présent essentiellement verticale, la distraction tend aujourd’hui à ouvrir de nouvelles possibilités izrâce à la mise en oeuvre de ce processus dans le sens horizontal. Avec un résultat plus prévisible que les autres thérapeutiques, une absence de morbidité du site donneur et une consolidation osseuse plus rapide, la distraction alvéolaire est, à l’heure actuelle, une technique d’aménagement parodontal à prendre en considération dans le cadre d’une réhabilitation orale implantaire.
