Prosthetic design and operative technique have been greatly improved since then, and there is now an accepted standard concept of total knee arthroplasty. Thirteen years after the London symposium, another international symposium on total knee replacement was held, this time in Nagoya, Japan, in 1987. Its ambitious objective was to push forward the frontiers of continuous investigation and improve ment of total knee replacement. The fruits of the individual efforts presented at the Nagoya symposium were published in a volume of proceedings entitled Total Knee Replacement. In the years since 1987, further investigations have been conducted in various parts of the world regarding prosthetic design, fixation, long-term radiological follow-up, biomechanical evaluation, and biomaterials research. In knee ligament reconstruc tion, rapid progress has been made in the past five years in clinical practice and fundamental research by means of arthroscopic surgery and tissue transplantation, and we have come close to establishing a standard treatment. Under these circumstances, an international symposium on knee joint reconstruc tion was planned for 1994, again to be held in Nagoya, to provide ample opportunity for exchanging information and sharing clinical experience from around the world.

There are two problems with this line of thinking. First of all, the structural changes that commonly take place in arthritis, things such as cartilage loss, meniscus tears, and bone spurs, can all be found in people with no knee pain. Consider the study that looked at 319 subjects between the ages of 25 and 74 with knee arthritis on x-ray - and found that only 47% had knee pain. Put another way, 53% of these people were walking around with knee arthritis and no pain. Yet another study looked at 49 subjects over the age of 45 with no knee pain or arthritis, and found that 76% of them had meniscus tears The second problem with focusing on the structural changes in knee arthritis is trying to figure out what's causing your pain. Most of the numerous structures in your knee have nerve pain fibers going to them, so good luck trying to pinpoint which one(s) are the exact source of your pain. In fact, about the only knee structure we can safely rule out is the articular cartilage that can be seen wearing out on x-rays Knee cartilage itself actually has no pain fibers going to it, and therefore cannot produce any pain. Book excerpt: Pathological conditions affecting the hip and knee joints occupy a particular place amongst the important orthopaedic entities affecting the extremities. On the one hand they are relatively frequent and on the other they mean for the patient limitation of his ability to walk, because of their considerably detrimental effects. A purposeful basic treatment of these joint diseases (and here osteoarthritis takes pride of place) is only possible if it stems from a reliable biomechanical analysis of the normal and patho logical stressing of the joint in question. Whilst the situation in the hip can be considered to be fundamentally clarified, a com prehensive representation of the knee is still lacking particularly when taking into account the latest knowledge of biomechanics. Recently our concepts of the kinematics of the knee have been completely changed, but the clinically important question of articular stressing remains unanswered. Dr. Maquet has carried out pioneer work in this field for some years in adapting, by analogy, to the knee joint, principles already accepted for the hip joint. Since the knee is not a ball and socket joint, a complicated problem arises for which new thoughts are necessary. The results of the numerous operations carried out by Dr. Maquet according to the biomechanical con siderations, demonstrate that his thinking is fundamentally correct. Above all, it is here again proven (as earlier in the case of the hip) that healing of osteoarthritis depends decisively on reducing and evenly distributing joint pressure.
