Available in PDF, EPUB and Kindle. Book excerpt: A treatise on operative neurosurgery by an officer of the Regular Army would ordi narily be expected to emphasize management of trauma. But this is far from an ordinary effort and the reader will quickly realize that here we encounter a Handbuch in the classic mold. General neurosurgical procedures are presented with great clarity by an extra ordinary individual. Originally an ornithologist of repute and accomplished student of medieval history, Colonel KEMPE received his residency training in the large and well-balanced neuro surgical program at the Walter Reed Army Medical Center. Later, he was assigned to that service and has been Chief of Neurosurgery at the Center since 1965. Through the years, he has made it a practice to sketch the steps and surgical anatomy of his operative proce dures for inclusion in the hospital records, and these drawings have been used on rounds with young house officers as one of the many notable features of the Army's neurosurgical residency program. His remarkable talent with the sketchbook coupled with his devotion to detail in patient care and in teaching has culminated in an opus which will be of tremendous value. The book is based upon the rich personal experience of the neurosurgeons at Walter Reed. The illustrations and text bring to the student an understanding of anatomy and the important minute details of surgical approaches which are unexcelled in any other presently-available text.

Book excerpt: This comprehensive, practical title invites all clinicians to take a fresh look at the evaluation and management of chronic daily headache (CDH). Developed by a distinguished international panel of experts, the book examines key social and economic issues around CDH and clarifies the diagnosis of CDH disorders, providing an understanding of the underlying biological substrates, offering guidance on the use of diagnostic testing and additional consultations, and outlining treatment strategies with the greatest potential to alleviate the burden of these patients and to provide the highest quality of care. The book fully examines the constellation of symptoms that constitute chronic daily headache, while also discussing the role of behavioral medicine and the important elements involved in taking a good history. The major forms and biology of chronic daily headache are covered, as is the role of diagnostic testing and treatment. The risk factors that lead patients to transform episodic primary headache disorders into the chronic form are examined. Invasive and neuromodulatory techniques are also discussed. A section on the classification of these disorders rounds out this important contribution to the literature. Chronic Headache – A Comprehensive Guide to Evaluation and Management will be of great interest to neurologists, primary care physicians, nurse practitioners, physician assistants, medical students, and other clinicians with an interest in chronic daily headache.

Available in PDF, EPUB and Kindle. Book excerpt: This supplement to "Acta Neurochirurgica" contains a selection of papers which were presented at the 9th Scientific Meeting of the European Society for Paediatric Neurosurgery on Space Occupying Lesions of the Cerebral Midline in Vienna, October 10-13,1984. This meeting was arranged at the same location where the ESPN was founded exactly seventeen years ago. Although the presentations in this meeting dealt with numerous important problems encountered in paediatric neurosurgery, the main emphasis was on that special problem which exemplifies the extraordinary advances in paediatric neurosurgery and its related fields. Therefore the main topic of this scientific meeting was dedicated to the subject of "Space Occupying Lesions of the Cerebral Midline". Recent diagnostic procedures, such as computerized axial tomography and magnetic resonance imaging, now enable the neurosurgeon preoperatively, to obtain precise data on the location, and in many cases also on the nature of a lesion deep within the brain. Fundamental new knowledge in neuroanatomy and neurotopography has now transformed previous high-risk procedures into routine ones for the neurosurgeon, and an abundance of new surgical techniques has improved the success rate in the treatment of many patients. The scientific meetings of the ESPN have proved to be a successful forum for the exchange of experiences, opinions and even critical discussions. The present selection of papers will undoubtedly support this endeavour. Wolfgang T. Koos Gerhard Pendl Contents A. Book excerpt: Technological progress in neurosurgery - preoperative investigation of the exact anatomy of the patient, detailed planning of the procedure, and use of endoscopes and videosurgery – have made approaches for intracranial microsurgical procedures smaller compared to historically standard neurosurgical approaches. Building on the previous works "Endoscopic Anatomy for Neurosurgery" and "Keyhole Concept in Neurosurgery," this book offers a systematic overview of keyhole approaches in the daily work of a neurosurgeon. The approaches, strategies, indications and technical details described here are complemented by anatomical pictures, schemes, and artists’ illustrations, and analyzed with regard to geometric boundaries and the topography of the target structures.

Available in PDF, EPUB and Kindle. Book excerpt: The region of the skull base was long considered a surgical barrier because of its complex anatomy. With few exceptions, the region immediately beyond the dura or bony skull base constituted a "no man's land" for the surgeon working from the other direction. A major reason for this was the high morbidity associated with operative procedures in that area using traditional dissection techniques. This situation changed with the advent of the operating microscope. Used initially by ear, nose and throat specialists for resective and reconstructive surgery of the petrous bone and parana sal sinuses, the operating microscope was later introduced in other areas, and neurosurgeons began using it in the mid-1960s. With technical equality thus established, the groundwork was laid for taking a new, systematic, and interdisciplinary approach to surgical problems of the skull base. Intensive and systematic cooperation between ear, nose and throat surgeons and neurologic surgeons had its origins in the departments of the University of Mainz bindly supported by our chairmen Prof. Dr. Dr. hc Kurt Schiirmann (Department of Neurosurgery) and Prof. Dr. W. Kley (Depart ment of Ear, Nose and Throat Diseases, Head and Neck Surgery). The experience gained from this cooperation was taught in workshops held in Hannover from 1979 to 1986, acquiring a broader interdisciplinary base through the participation of specialists from the fields of anatomy, patholo gy, neuroradiology, ophthalmology, and maxillofacial surgery.

Book excerpt: This didactic book clearly and systematically describes the anatomical-surgical fundamentals of cranial neurosurgery, relating them to norm variants, imaging modalities and interdisciplinary aspects. All illustrations, hand drawn in ink by the first author, are simple and self-explanatory. The book reflects the first author’s lifetime experience as an academic neurosurgeon and teacher, as well as the second author’s theoretical and practical knowledge of neurosurgical subspecialties such as epilepsy surgery. In addition to its core audience in neurosurgery, it provides all connected disciplines, in particular neuroradiology, neurology, neuropathology, ENT surgery, maxillofacial surgery and eye surgery, with unique anatomical insights into the neurosurgeon’s perspective.
