Book excerpt: More than 20 years have passed since the International Symposium on Total Knee Replacement was held in London in 1974. Prosthetic design and operative technique have been greatly improved since then, and there is now an accepted standard concept of total knee arthroplasty. Thirteen years after the London symposium, another international symposium on total knee replacement was held, this time in Nagoya, Japan, in 1987. Its ambitious objective was to push forward the frontiers of continuous investigation and improve ment of total knee replacement. The fruits of the individual efforts presented at the Nagoya symposium were published in a volume of proceedings entitled Total Knee Replacement. In the years since 1987, further investigations have been conducted in various parts of the world regarding prosthetic design, fixation, long-term radiological follow-up, biomechanical evaluation, and biomaterials research. In knee ligament reconstruc tion, rapid progress has been made in the past five years in clinical practice and fundamental research by means of arthroscopic surgery and tissue transplantation, and we have come close to establishing a standard treatment. Under these circumstances, an international symposium on knee joint reconstruc tion was planned for 1994, again to be held in Nagoya, to provide ample opportunity for exchanging information and sharing clinical experience from around the world.

Myths Several medical myths scream for attention; while some are harmless, many are downright dangerous. This may be due to the lack of public awareness or even the fear associated with surgery. These myths result in many patients undergoing different forms of alternative therapies like Ayurveda, homeopathy, magnetotherapy, electrotherapy, and several other remedies, which are not curative. After going through the ordeal, the sad part is that they end up suffering from more pain, feeling depressed, and finally will be in need of a more complicated corrective operation. Arthritis has become one of the common problems across the world, affecting people of all age groups. There are more than a hundred types of arthritis, and most of them are not curable through medicines and other therapies. Hence, joint replacement surgery becomes inevitable for patients suffering from end-stage joint diseases, for they deal with severe pain, deformity, and immobility. That said, those patients who have mild arthritis can be relieved of their discomfort with minimal support. For this, however, patients need effective education and assistance to simplify their treatment plans. Putting things in perspective, joint replacement surgeries are one of the most common operations performed across the world, and in the last fifty years, the demand for this has gone through the roof. Patients who are otherwise crippled with harrowing pain are now witnessing tremendous improvement in their quality of life. Future There is a sharp increase in the number of patients diagnosed with arthritis each year across the world. Arthritis is more common in older people, and the number seems to be growing exponentially. In fact, a study by the Centers for Disease Control (CDC) for the National Arthritis Data Workgroup (NADW) had revealed that by 2030, the number of people with arthritis or any other rheumatologic condition would increase by a whopping 44% a surge to 67 million! In this book on "Joint Replacements - A Patient's Handbook," I have answered over a hundred commonly asked questions by my patients. My belief in educating the population on preventive strategies, the merits of several methods of treatments, and their complications has indeed come true with this book. Book excerpt: The author's practice has identified a common but little recognised cause of knee osteoarthritis (knee OA), and in 2004 he devised an arthroscopic treatment option called arthroscopic cartilage regeneration facilitating procedure (ACRFP) that has, without using artificial knees, (1) relieved thousands of patients of their knee pain without using analgesics, (2) helped open up the joint space in their knees, and (3) improved their mobility. The ACRFP has debunked the myth that cartilage in the knee naturally degenerates and proved that "degenerative arthritis of the knee" is a misnomer. Since 2007, his practice has further expanded the ACRFP into a comprehensive, multidisciplinary management protocol-the knee health promotion option (KHPO)-to treat patients with knee OA. The result shows the effectiveness of the KHPO in treating knee OA. Thousands of people have therefore kept their knees under this KHPO. This common but little recognised cause of knee OA is based on the work of his research team that has spanned 15 years. They have published 12 scientific papers on this research.
