Starting in the 1970s evidence began to accumulate that the etiologic agent of OPA is a retrovirus, culminating in the deduction of the sequence of a novel retrovirus associated with the disease, jaagsiekte sheep retrovirus (JSRV). Very recently, substantial progress has been made in JSRV research, including isolation of infectious and oncogenic molecular clones of JSRV, identification and cloning of the JSRV receptor, and demonstration that the envelope gene of JSRV functions as an oncogene. This book is a consensus volume that comprehensively reviews all aspects of JSRV research, including natural history of the disease, histopathology of OPA, molecular biology of JSRV, oncogenic transformation by JSRV, and comparison with human lung cancer.

Initially, studies on oncogenic animal retroviruses provided important insights into molecular processes in carcinogenesis – most notably the existence and mechanisms of action of oncogenes and proto-oncogenes. Moreover, several human diseases are caused by retroviruses, including AIDS, adult T-cell leukemia and the neurological disease HAM/TSP. The topic of this volume is a relatively unknown animal retrovirus, jaagsiekte sheep retrovirus, the causative agent of transmissible lung cancer in sheep –ovine pulmonary adenocarcinoma. The disease was first documented in South Africa in the 1800s, it has a wide geographical distribution, and it is of economic importance in high endemic regions. However, until very recently the nature of the etiologic agent was unclear, and relatively few laboratories actively studied the disease.

Paradoxically, both hyaluronan (HA) and hyaluronidases, the enzymes that eliminate HA, have also been correlated with cancer progression. Hyaluronan, a long-chain polymer of the extracellular matrix, opens up tissue spaces through which cancer cells move and metastasize. It also confers motility upon cells through interactions of cell-surface HA with the cytoskeleton. Embryonic cells in the process of movement and proliferation use the same strategy. It is an example of how cancer cells have commandeered normal cellular processes for their own survival and spread. There are also parallels between cancer and wound healing, cancer occasionally being defined as a wound that does not heal. The growing body of literature regarding this topic has recently progressed from describing the association of hyaluronan and hyaluronidase expression associated with different cancers, to understanding the mechanisms that drive tumor cell activation, proliferation, drug resistance, etc. No one source, however, discusses hyaluronan synthesis and catabolism, as well as the factors that regulate the balance. To bridge the gap among these diverse groups of investigators, we conceived of a book covering a broad spectrum of the basic scientific, clinical, and pathological aspects of diseases associated with papillomaviruses. Although the principal thrust of this book is directed at human papillomaviruses, fundamental knowledge of animal viruses is essential to the current understand ing of the molecular mechanisms of cell transformation. For this reason, a chapter on animal viruses has also been included. Some of the experimental work having to do with the elucidation of transformation and other aspects of interaction between the virus and the cell cannot be based on human papillomaviruses because of a lack of suitable experimental models. Hence, some of the chapters dealing with fundamental aspects of viral molecular biol ogy are based on animal models. We were very fortunate in having persuaded a number of distin guished colleagues to contribute to this work.

Book excerpt: This edition of ICD-O, the standard tool for coding diagnoses of neoplasms in tumour and cancer registrars and in pathology laboratories, has been developed by a working party convened by the International Agency for Research on Cancer / WHO. ICD-O is a dual classification with coding systems for both topography and morphology. The book has five main sections. The first provides general instructions for using the coding systems and gives rules for their implementation in tumour registries and pathology laboratories. Section two includes the numerical list of topography codes, which remain unchanged from the previous edition. The numerical list of morphology codes is presented in the next section, which introduces several new terms and includes considerable revisions of the non-Hodgkin lymphoma and leukaemia sections, based on the WHO Classification of Hematopoietic and Lympoid Diseases. The five-digit morphology codes allow identification of a tumour or cell type by histology, behaviour, and grade. Revisions in the morphology section were made in consultation with a large number of experts and were finalised after field-testing in cancer registries around the world. The alphabetical index gives codes for both topography and morphology and includes selected tumour-like lesions and conditions. A guide to differences in morphology codes between the second and third editions is provided in the final section, which includes lists of all new code numbers, new terms and synonyms added to existing code definitions, terms that changed morphology code, terms for conditions now considered malignant, deleted terms, and terms that changed behaviour code.

It would be ideal to be able to detect and treat preinvasive bronchial lesions before progressing to invasive cancer. Advances in early diagnostic and treatment options have the potential to manage LC while still in an intraepithelial and/or microinvasive stage. WLB is one of the most commonly used diagnostic tools for LC. However, WLB is limited in its ability to detect small intraepithelial and microinvasive preinvasive lesions.AFB was developed to address this limitation by WLB. Indications for AFI examination; (1) Patients with known or suspected LC (2) Patients with abnormal sputum cytology (3) Patients after curative surgery for stage I LC(4) Current or former smokers with symptoms. A total of 74.7% patients had abnormal sputum cytology, 14.3% had known or suspected LC. All patients underwent conventional WLB followed by AFI examination; using the AFI device. Bronchoscopic-guided biopsy specimens were obtained. Sensitivity, specificity, and diagnostic accuracy of WLB versus that of AFI, were 58.3% Vs 88.9%; 46.7% Vs 71.9%; and 53.0% Vs 80.9%. with a very high significant difference (p value
