Book excerpt: In response to a request from the Office of Science and Technology Policy and the Office of the Assistant Secretary for Preparedness and Response, the National Academies of Sciences, Engineering, and Medicine convened a standing committee of experts to help inform the federal government on critical science and policy issues related to emerging infectious diseases and other 21st century health threats. This set of Rapid Expert Consultations are the first of their kind and represent the best evidence available to the Committee at the time each publication was released. The science on these issues is continually evolving, and the scientific consensus the Committee reaches on these topics will likely evolve with it. The standing committee includes members with expertise in emerging infectious diseases, public health, public health preparedness and response, biological sciences, clinical care and crisis standards of care, risk communication, and regulatory issues.

Book excerpt: The proceedings of the National Consultation on Safety and Force held on December 11-13, 1979, are presented in the report. The consultation highlights the concern of the Community Relations Service, an arm of the Department of Justice, with the issue of alleged police use of excessive force. The consultation objectives were to identify key issues and problems related to police-minority community relations, to explore alternatives and solutions to the problem, and to develop a working trilateral (black, Hispanic, police) relationship geared toward crisis response and problem-solving. The opening remarks addressed the need for an approach to developing a better understanding between minorities and law enforcement agencies. Four case studies in Colorado, California, Washington, and Texas, were presented to illustrate successful community-police efforts to increase safety and reduce the use of police force. A small-group session considered eight questions regarding issues in race relations, police use of force, and options to reduce the use of force. Panel discussions considered options for both the police and for community groups on reducing tension and conflict which could lead to the use of excessive force. Other panel discussions considered special problems in municipal liabilities which arise from the increasing number of civil suits based on alleged police brutality, special problems of nonurban police departments, and the obstacles and opportunities to progress in the area of police use of excessive force. An audio-visual presentation provided a training demonstration of police officers under stress.

Book excerpt: In 1996, the Institute of Medicine (IOM) released its report Telemedicine: A Guide to Assessing Telecommunications for Health Care. In that report, the IOM Committee on Evaluating Clinical Applications of Telemedicine found telemedicine is similar in most respects to other technologies for which better evidence of effectiveness is also being demanded. Telemedicine, however, has some special characteristics-shared with information technologies generally-that warrant particular notice from evaluators and decision makers. Since that time, attention to telehealth has continued to grow in both the public and private sectors. Peer-reviewed journals and professional societies are devoted to telehealth, the federal government provides grant funding to promote the use of telehealth, and the private technology industry continues to develop new applications for telehealth. However, barriers remain to the use of telehealth modalities, including issues related to reimbursement, licensure, workforce, and costs. Also, some areas of telehealth have developed a stronger evidence base than others. The Health Resources and Service Administration (HRSA) sponsored the IOM in holding a workshop in Washington, DC, on August 8-9 2012, to examine how the use of telehealth technology can fit into the U.S. health care system. HRSA asked the IOM to focus on the potential for telehealth to serve geographically isolated individuals and extend the reach of scarce resources while also emphasizing the quality and value in the delivery of health care services. This workshop summary discusses the evolution of telehealth since 1996, including the increasing role of the private sector, policies that have promoted or delayed the use of telehealth, and consumer acceptance of telehealth. The Role of Telehealth in an Evolving Health Care Environment: Workshop Summary discusses the current evidence base for telehealth, including available data and gaps in data; discuss how technological developments, including mobile telehealth, electronic intensive care units, remote monitoring, social networking, and wearable devices, in conjunction with the push for electronic health records, is changing the delivery of health care in rural and urban environments. This report also summarizes actions that the U.S. Department of Health and Human Services (HHS) can undertake to further the use of telehealth to improve health care outcomes while controlling costs in the current health care environment.

"Following a decade of armed conflict that led to the virtual collapse of the country's social, economic, legal and political fabric, the Sierra Leone National Consutation on women and men in partnership for post-conflict reconstruction ... brought together 250 people from governmental and non-governemental organisations to discuss ways in which the war has impacted differently on women, children and men and how best to ensure gender equality in all reconstruction efforts."

Available in PDF, EPUB and Kindle. Book excerpt: This study examines the role and the value of prior consultation among nations in international law. International disputes frequently occur when one nation, with no hostile intent, takes unilateral action that adversely affects the interests of other nations. It is generally acknowledged that some of these disputes could be avoided, and others could be ameliorated, if the acting government would assess beforehand the risk of harm to other nations. The most effective way to do this is through prior consultation with representatives of potentially affected nations. When governments are able to act unilaterally, they have very little incentive to refrain from taking self-interested action in order to consider the adverse interests of other nations. Thus, it is important to determine the circumstances in which international law imposes on them a duty to consult. The author examines these determining circumstances in detail.
