Book excerpt: Cardiac resynchronization therapy (CRT) is one of the most exciting new advances in the treatment of chronic severe (NYHA symptom class) heart failure associated with dyssynchronous ventricular contraction that is refractory to medical treatment. In all randomized trials CR has resulted in improved NYHA symptom class, exercise capacity and quality

Book excerpt: Hypertension remains the leading cause of cardiovascular morbidity and mortality in spite of current medical therapies. It has been estimated that 50% of Western civilization has hypertension and approximately 20% of patients have resistant hypertension. Renal denervation (RDN) is a minimally invasive, endovascular catheter based procedure using radiofrequency ablation aimed at treating resistant hypertension. Early studies show a high degree of effectiveness in renal denervation to treat hypertension. This book examines renal pathophysiology and the rationale for renal denervation, as well as possible long term benefits and risks of this new therapy. The myriad of devices involved in the evolution of this therapy are discussed and the book concludes with analyses of the cost effectiveness and future applications.

Book excerpt: Percutaneous left atrial appendage (LAA) closure is an emerging technology for thromboembolic prevention in patients with atrial fibrillation (AF). The first human implantation of an LAA device occurred in 2001, and since then four devices have received CE mark approval. These devices are being widely used in Europe for LAA closure in patients who are poor candidates for long-term oral anticoagulation. In the US, the WATCHMAN device (Boston Scientific) is anticipated to receive FDA approval imminently for AF patients who are warfarin-eligible. This approval is projected to significantly expand the indications for LAA closures worldwide. Thus, the volume of procedures is anticipated to escalate. This book discusses the epidemiology of AF as a cause of stroke; the use of LAA closure in the reduction of thromboembolism with AF; early surgical approaches and novel surgical devices for LAA closure; and current percutaneous approaches and devices available for LAA closure. The emphasis of this book is on percutaneous technical approaches and contemporary trial results on the leading devices (PLAATO, WATCHMAN, Amplatzer Cardiac Plug, and LARIAT). It also reviews unapproved devices in development, in both clinical and pre-clinical phases.

Available in PDF, EPUB and Kindle. Book excerpt: The significantly updated second edition of this important work provides an up-to-date and comprehensive overview of cardiovascular magnetic resonance imaging (CMR), a rapidly evolving tool for diagnosis and intervention of cardiovascular disease. New and updated chapters focus on recent applications of CMR such as electrophysiological ablative treatment of arrhythmias, targeted molecular MRI, and T1 mapping methods. The book presents a state-of-the-art compilation of expert contributions to the field, each examining normal and pathologic anatomy of the cardiovascular system as assessed by magnetic resonance imaging. Functional techniques such as myocardial perfusion imaging and assessment of flow velocity are emphasized, along with the exciting areas of artherosclerosis plaque imaging and targeted MRI. This cutting-edge volume represents a multi-disciplinary approach to the field, with contributions from experts in cardiology, radiology, physics, engineering, physiology and biochemistry, and offers new directions in noninvasive imaging. The Second Edition of Cardiovascular Magnetic Resonance Imaging is an essential resource for cardiologists and radiologists striving to lead the way into the future of this important field.
