Le rôle de la vulnérabilité psychique parentale dans la genèse des troubles des interactions précoces et leurs conséquences sur le développement de l'enfant est suffisamment documenté pour qu'une prise en charge spécifique s'organise dans une continuité de l'anténatal au postnatal, en intégrant le repérage des vulnérabilités psychologiques dans le suivi médical le plus précocement possible. Préparer avec les parents les meilleures conditions possibles pour la venue au monde de leur enfant est un objectif nécessitant une pluridisciplinarité incontournable. Pendant la grossesse, des processus psychologiques particuliers rendent les futurs parents plus sensibles à une action de prévention et font de cette période un moment privilégié d'accessibilité au soin psychique. Un Entretien Prénatal Précoce proposé systématiquement à la femme enceinte et au couple autour du quatrième mois de grossesse a été instauré par le Plan Périnatalité 2005-2007. Réalisé majoritairement pas les sages-femmes dans le cadre du suivi obstétrical, il a pour objectif d'ouvrir le dialogue avec les parents, de recueillir leurs attentes et de chercher d'éventuels facteurs de vulnérabilité, afin d'adapté au mieux l'accompagnement proposé.Nous avons réalisé au CHU de Montpellier une étude qualitative évaluant l'impact de l'entretien prénatal précoce sur le parcours de soin des femmes enceintes porteuses d'une vulnérabilité psychique ayant consulté dans le service de pédopsychiatrie en comparant deux groupes, l'un ayant bénéficié d'un entretien prénatal précoce et l'autre n'en ayant pas bénéficié. Les objectifs secondaires étaient d'évaluer l'impact de l'EPP sur la morbidité obstétrico-pédiatrique, sur la dépression du post-partum et les troubles de la relation mère-bébé ainsi que sur les collaborations médico-psychologiques s'y rapportant. L'EPP constitue un outil majeur dans l'ouverture du dialogue avec les parents pour l'ajustement du suivi de grossesse grâce à un repérage précoce des vulnérabilités et des besoins tant du côté des parents que des professionnels. La singularité de chaque situation peut alors être prise en compte au sein d'un suivi pluridisciplinaire. La mise en place d'une coordination des orientations nécessaires par un professionnel de la grossesse afin d'anticiper les conditions de naissance et la rencontre des parents avec l'enfant est un axe prioritaire que cette recherche souligne. Actuellement, rares sont les études évaluant l'efficacité de cet outil, elles nécessiteraient d'être quantitatives afin de mettre en évidence une amélioration significative
