Book excerpt: Cognitive contextual framework (Grych & Fincham, 1990) and emotional security theory (Cummings & Davies, 1994) posit that children's appraisals about interparental conflict influence child adjustment outcomes. Though children exposed to intimate partner violence (IPV) involving their parents are at increased risk for developing trauma symptoms as well as dysregulated hypothalamic-pituitary-adrenal (HPA) axis functioning, the cognitive precursors to these outcomes have rarely been studied. The present study sought to investigate children's threat and self-blame appraisals about interparental conflict as potential mechanisms contributing to these adverse outcomes. The influence of child and family level contextual factors (gender; parent-child relationships) were also examined. The sample consisted of 119 10-year-olds and their mothers who were recruited from the community as a part of a larger longitudinal study of IPV. Though children's reports of IPV exposure in the past year were not associated with symptoms of posttraumatic stress disorder (PTSD), greater exposure to parental IPV was associated with attenuated cortisol output during the Trier Social Stress Test for children. Greater IPV exposure was also associated with increased threat appraisals, as well as worse parent-child relationship quality. Indirect effects of IPV on PTSD symptoms and HPA axis responsivity through children's appraisals were not significant. These results suggest that witnessing parental IPV is threatening for children and can have negative consequences for parent-child relationships. In addition, this study highlights the complexity of factors that influence the development of trauma symptoms following exposure to parental IPV.

Book excerpt: The Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition [DSM-IV; American Psychiatric Association (APA), 1994] alludes to the importance of cultural influence in establishing diagnoses. Nevertheless, culture has received little attention in studies of posttraumatic stress symptomatology (PTSS) in maltreated children. Additionally, some studies have shown that sexually abused (SA) children may be at greater risk for developing PTSS than victims of physical abuse (PA) or combined abuse (PA and SA). However, interpretations are complicated by the frequent overrepresentation of girls to boys in SA groups, small sample sizes, and inconsistent maltreatment classification methods. To address these issues, this study aimed to establish concurrent validity for the CBCL-PTSD scale and its youth self-report version (YSR-PTSD), examine the unique effects of SA on PTSS, and determine the effects of gender by type of maltreatment on PTSS across ethnicity. Hypotheses: (1) Hispanics would evidence greater PTSS than non-Hispanics; (2) Girls would evidence greater PTSS than boys; (3) SA Hispanic girls would evidence greater PTSS than children of other ethnic groups; and (4) SA would contribute significantly to PTSS (controlling for age, gender, and foster care placement). Preliminary Findings: Due to low representation, SA boys were dropped from the sample, therefore, Hypothesis 2 could not be tested. Study 1: Archival data were analyzed for 175 girls from an outpatient mental health clinic. Book excerpt: Women exposed to intimate partner violence (IPV) during pregnancy are at increased risk for compromised mental health. There is evidence that trauma, both violent and non-violent events, are associated with the development of posttraumatic stress disorder (PTSD) and depression. Routine assessment for conditions such as depression and IPV is recommended for women of reproductive age, but the current health care environment impedes the ability of clinicians to complete recommended screenings for every patient. The purpose of this cross-sectional study was to examine trauma, IPV type and IPV severity as predictors of posttraumatic stress disorder and depression among low-income women who experienced intimate partner violence during pregnancy. A secondary purpose was to examine the validity of a brief screening measure of self-rated mental health (SRMH) to identify PTSD and depression in comparison to previously established mental health assessment measures. The baseline data of 239 low-income pregnant women who experienced recent IPV was analyzed to address the study objectives. Univariate and bivariate analyses were used to describe the sample and identify relationships among the major study variables. Regression models were developed to further explore these relationships to identify predictors of PTSD and depression. Calculation of validity coefficients and receiver operating characteristic (ROC) analyses were conducted to determine the ability of SRMH to identify depression and PTSD among these women. PTSD was identified in 40% of the women, while 41% had high risk for depression. Age was the only predictor of PTSD development, with women more likely to develop PTSD as they aged. Women who experienced severe psychological IPV were more likely to have high risk for depression than those exposed to severe sexual or physical IPV. The SRMH item demonstrated moderate validity and accuracy in its ability to detect high risk for depression, suggesting it may be a useful way to increase screening and identification of women at risk for depression and who may benefit from a more comprehensive mental health assessment. These results suggest that current mental health screening measures should be expanded to routinely assess trauma, as well as IPV, to increase early identification and treatment of women most at risk for depression and PTSD. The current clinical climate may not be conducive to completing the recommended routine screenings for each patient. The present study findings indicate that single item measures may be useful, but further research is needed to establish a valid measure of mental health that is quick and efficient in a busy clinical setting.

Book excerpt: Research with military veterans has long recognized the role of perpetrating violence on subsequent PTSD and guilt, particularly with regard to moral injury. In the context of intimate partner violence (IPV), mental health consequences of perpetration have been minimally examined and cross-sectional associations have been interpreted to mean that PTSD and shame (a construct closely related to guilt) precipitate perpetration. However, theory and research regarding moral injury, as well as the dyadic nature of IPV, suggest that the opposite directionality may also exist. In the absence of sufficient longitudinal data, I examined differential associations between IPV perpetration and PTSD/guilt for parents and non-parents. Parenting status was examined as a moderator because it was thought that parents would be more susceptible to moral injury due to a greater multitude of potential consequences of IPV perpetration (e.g., child witnessing perpetration). I conducted multilevel regression analyses to examine PTSD and self-reported guilt related to physical IPV perpetration among 64 heterosexual community couples (N = 128 individuals). IPV perpetration was significantly associated with PTSD symptom severity, but not guilt. Parenting status moderated the relationship between IPV perpetration and overall guilt, state guilt, and trait guilt, such that non-parents who perpetrated more IPV experienced higher levels of guilt while parents did not. Parenting status did not moderate the relationships between IPV victimization and PTSD or guilt, suggesting the pattern of results is specific to IPV perpetration. Findings did not necessarily support or disconfirm the proposed theory regarding the emotional consequences of perpetration. Interestingly, however, results suggest that parents may exhibit a muted guilt response in relation to their perpetration that warrants future investigation.

Book excerpt: Although a good deal of research has been conducted examining the effects of parental psychopathology of various types (i.e. maternal and paternal depression and schizophrenia, paternal PTSD), very few studies have investigated the influence of maternal PTSD on mothers' parenting and their children's behavioral and psychological functioning. This paucity of research is in spite of the fact that women exhibit higher lifetime PTSD prevalence rates than men. The current study examined the influence of maternal PTSD and trauma on mothers' parenting, family functioning, and children's psychological well-being in a sample of 125 mothers and 34 mother-child dyads. Mothers provided self-report information regarding their lifetime history of traumatic events, current PTSD symptoms, current parenting beliefs, and beliefs about the current functioning of their family. Mothers also reported on the current psychological functioning of their oldest child. Analyses revealed that mothers in the PTSD group reported significantly worse parenting satisfaction and efficacy, and family problem-solving, communication, and general functioning than did mothers in the Non-trauma group. Mothers in the PTSD group also reported that their children suffer from significantly more internalizing and total psychological problems than children of mothers in the Non-trauma group. Mothers who had experienced a trauma but did not suffer from PTSD reported significantly worse family problem-solving and child total problems than mothers in the Non-trauma group. These results suggest that although a lifetime history of traumatic experiences may negatively impact mothers' parenting, family functioning, and the psychological well-being of their children to a slight degree, the presence of PTSD following these experiences is even more detrimental.
