Book excerpt: Background: A growing body of literature indicates that women, in comparison to men, report significantly more barriers to exercise, often times related to perceived lack of time due to their social roles (e.g. family responsibility, childcare). Therefore, it has been considered that exercise programs that require less time commitments would be more attractive for women to alleviate this concern. Objective: This review was conducted to comprehensively understand the present literature surrounding women and such a time-efficient protocol: high intensity interval training (HIIT). The primary aim was to determine the impact of HIIT on both cardiorespiratory fitness (CRF) and body composition. The secondary aim was to identify what variables, if any, influence the effect of HIIT on changes in CRF and body composition. Methods: A database search was conducted in MEDLINE (PubMed), CENTRAL (Cochrane), and ESCO (SPORTdiscus) to obtain all peer-reviewed publications up until November 2020. Studies were included if HIIT protocols were aerobic-based, a minimum of two weeks in duration, participants were between the ages of 18-65 years and free from cardiovascular, metabolic, or renal disease and cancers. Comparisons were made for changes in maximal or peak oxygen consumption (VO2max or VO2peak), fat mass (FM), fat-free mass (FFM), and body fat percentage (BF%). Results: 41 studies were included in this review, involving a total of 628 female subjects who were predominantly younger (582 subjects between ages 18-35 years) and overweight (body mass index ranging from 20.3-35.7 kg-m-2). Exercise intervention duration ranged from 2-15 weeks (7.8±3.7 weeks), utilizing between 2-5 sessions (3.1±0.4 visits) per week. Of the 37 studies with CRF as an outcome measures, 34 reported improvements. 35 studies reported pre- and post-intervention assessments surrounding body composition. 14 determined significant improvements in BF% and 12 studies reported a significant decrease in FM, six showed significant increases in FFM. Conclusion: It is suggested that HIIT can significantly improve CRF in women, irrespective of age, training status, BMI, and training frequency. However, exercise intervention duration, in conjunction with the appropriate intensity of exercise may influence the improvements. HIIT interventions that are shorter in duration (≤7 week) should consider utilizing intensities >100% VO2max (supramaximal) to elicit improvements in CRF. Intensities ranging from 85-95% VO2max are sufficient to augment CRF in interventions exceeding 7 weeks. The effect of HIIT on body composition is less clear. Excess post-exercise oxygen consumption (EPOC) and energy expenditure greatly exceeding 1000 MET-minutes per week may account for reductions in FM. However, there is no guarantee that these reasons alone account for all reductions in FM. There were improvements in FFM across very few studies (n = 6). It is suggested that aerobic-based HIIT be supplemented with resistance training to generate superior improvements in FFM. Furthermore, lack of controlled diet across all studies makes it difficult to determine if caloric intake could account for any changes in body composition. While HIIT may be an effective and time-efficient protocol for women, future research should focus on ecological validity and women’s adherence to such vigorous training, specifically within non-athletic and sedentary subgroups.

Book excerpt: The purpose of this study was to determine the effects of following either the Curves® Fitness and Weight Management Plan or the Weight Watchers® Momentum Plan on body composition and markers of health and fitness in previously sedentary obese women. Fifty-one women (age 35±8 yrs; height 163±7 cm; weight 90±1 kg; BMI 34±5 kg/m2; 47±7% body fat) were randomized to participate in the Curves® (C) or Weight Watchers® (W) weight loss programs for 16-wks. Participants in the C group (n=24) followed a 1,200 kcal/d diet for 1-wk; 1,500 kcal/d diet for 3 wks (~30%:45% CHO:PRO); and 2,000 kcals/d for 2-wks (45:30) and repeated this diet while participating in a supervised Curves® with Zumba program 3-d-wk. Remaining subjects (n=27) followed the W point-based diet program, received weekly group counseling, and were encouraged to exercise. Body composition, anthropometrics, resting energy expenditure (REE), lipid biomarkers, and hormone concentrations were assessed at 0, 4, 10, and 16 weeks. Maximal cardiopulmonary exercise capacity and upper and lower body isotonic strength and endurance were assessed at 0 and 16 weeks. Data were analyzed using multivariate analysis of variance for repeated measures. MANOVA analysis of body composition data revealed overall time (Wilks' Lamda p=0.001) and time by diet effects (Wilks0́9 Lamda p=0.003). Subjects in both groups lost a similar amount of total mass (C -2.4±2.0, -4.1±3.4, -5.1±3.9; W -2.3±2.3, -4.5±3.0, -5.5±4.6 kg, p=0.78). However, subjects in the C group tended to have a greater reduction in percent body fat (C -3.3±5.2, -3.2±4.6, -4.7±5.4; W 0.6±6.7, -0.6±8.3, -1.4±8.1%, p=0.10) and body fat mass (C -3.9±5.5, -4.6±5.3, -6.4±5.9; W -0.4±5.7, -2.1±6.7, -2.9±7.8 kg, p=0.09), while maintaining FFM (C 1.5±4.3, 0.52±3.7, 1.3±4.0; W -1.8±5.4, -2.4±5.8, -2.5±5.1, p=0.01). While both groups had increases cardiovascular fitness, the C group experienced improvements in upper body muscular endurance (C 1.4±3.9; W -1.2±2.4 repetitions, p=0.006). Both groups experienced improvements in lipid biomarkers; however, only the C group experienced a moderate increase in HDL-c. Results indicate that participants following the C program experienced more favorable changes in body composition and markers of fitness and health than participants in the W program. Pregnancy may be one of the causes of this, with 56% of women of childbearing age being overweight or obese. Excessive weight gain during pregnancy and postpartum weight retention may increase a woman's risk of obesity and chronic disease later in life. Moderate calorie restriction and exercise interventions have been shown to reduce body weight and improve body composition during the postpartum period. While weight loss interventions have been successful, high attrition rates limit the widespread effectiveness of these interventions. Furthermore, there is a lack of research examining the effects of a weight loss program on cardiometabolic risk factors in this population. Therefore, the primary aim of the studies in this dissertation were: 1) To determine the effect of a diet and exercise intervention on weight, body composition, and cardiometabolic risk factors, 2) to improve lifestyle behaviors through improved diet quality and cardiovascular fitness, and 3) to describe the relationship between chronic inflammation and bone mineral density (BMD) in overweight and obese postpartum women. The first study concluded that a home-based diet and exercise program resulted in greater reductions in weight, waist circumference, sagittal diameter, and abdominal fat mass compared to a control group. Additionally, the intervention resulted in a significant improvement in cardiovascular fitness. The second study concluded that the intervention improved triglyceride concentrations, decreased insulin resistance, and improved metabolic syndrome risk factors. Finally, the third study concluded that the intervention resulted in less loss of BMD at the total body and hip and less loss of BMC at the hip, lumbar spine, and femoral neck after controlling for lactation status and weeks postpartum at baseline. Change in cardiovascular fitness was a predictor of change in total hip and femoral neck BMD, while change in inflammation was a predictor of change in total body BMD. These results suggest that a home-based, diet and exercise intervention is effective in improving body composition, cardiovascular fitness, and some cardiometabolic risk factors in overweight and obese postpartum women. It is possible that this is due to either a decrease in spontaneous daily physical activity or increased caloric intake as a result of participation in this program.

In adults, the consumption of dairy foods, as part of a lifestyle intervention, can promote favourable body composition changes. However, results of the few studies examining the combined effects of exercise and dairy consumption on body composition in adolescents are inconclusive. The purpose of our study was to determine whether increased dairy consumption, along with structured exercise training and dietary guidance within a weight management program, can promote favourable changes in body composition, anthropometry and cardiovascular fitness in overweight/obese adolescent females. Sixty-one adolescent females (age: 14.8±2.2 y; BMI: 29.3±5.1 kg/m2) were randomized to 3 groups: recommended dairy (RDa; n=24); low dairy (LDa; n=22); control (GCon; n=8), and 54 participants completed the study. The RDa and LDa groups participated in a 12-week, individualized, eucaloric, lifestyle modification intervention consisting of mixed-mode exercise 3x/week, and 5 nutritional counselling sessions with a registered dietitian. RDa group was provided 4 servings/day of dairy (as milk, Greek yogurt and cheese), while LDa maintained habitually low intakes of 0-2 servings/day. Seven-day food records were collected at weeks 0 and 12. Body composition, waist/hip circumference, and VO2peak were assessed for all groups at weeks 0 and 12. Both RDa and LDa decreased body fat (-1.7±1.5%, -1.2±1.1%, respectively), fat mass (-1.3±2.1kg, -1.1±2.0kg, respectively) and subcutaneous fat thickness (-12.5±10.0mm, -9.1±9.4mm, respectively) compared with the GCon (0.3±1.3%, 0.8±1.8kg, 3.0±8.0mm, respectively) (p ≤0.002 for all). RDa decreased fat mass (kg) more than LDa (p=0.001). Both RDa and LDa gained more lean mass (1.5±1.9kg, 0.7±1.6kg, respectively) than GCon (0.5±1.4kg). RDa increased lean mass more than LDa (p≤0.001). VO2peak (ml/min/kg) did not differ between groups following the intervention (p=0.093). These findings suggest that the inclusion of a variety of dairy foods in the diet of overweight/obese adolescent girls, as part of a weight management intervention program, is beneficial to the overall improvement in body composition.

The report concludes that selected cardiorespiratory endurance, musculoskeletal fitness, and body composition measures should be in fitness surveys and in schools. Collecting fitness data nationally and in schools helps with setting and achieving fitness goals and priorities for public health at an individual and national level.

Book excerpt: ABSTRACT: Background: The purpose of this project was to review the best evidence to determine an intervention to treat cardiopulmonary compromise in obese women. The research question inquired if weight loss through diet compared to weight loss through exercise would provide a greater effect on the cardiopulmonary function in obese women. Method: A thorough review of literature was conducted. Titles and abstracts were compared to an inclusion and exclusion criteria that were developed based on the research question and specific elements of the patient case. Included articles were evaluated and appraised and the results were integrated to formulate a clinical decision. Results: Seven articles were used to address the patient case. Five of the 7 articles concluded that only subjects who performed aerobic exercise as part of their weight loss program had an increase in cardiopulmonary function as measured by maximal oxygen consumption. Discussion/Conclusion: Upon review of the literature, aerobic exercise alone can have a significant improvement in maximal oxygen consumption in obese females when done for at least 8 weeks, 3-5 days a week, in 25-40 minute intervals. Additional diet protocols developed by a certified nutritionist may aid in a change in body mass, but has no significant effects on the cardiorespiratory fitness. Exercise has little impact on the amount of weight lost during a weight loss program. However, it does significantly increase the physiological benefits of weight loss and the cardiorespiratory health of patients. The main limitation of this review was that these findings were found in obese women without any significant comorbidities.
