Plusieurs études et rapports de cas tentent d’apporter la preuve de cette relation. Le développement d’un sepsis articulaire dépend de nombreux facteurs ; certains patients semblent plus susceptibles à l’infection que d’autres. Il n’existe pas de directives officielles concernant la prise en charge des patients porteurs d’implant articulaire, et notamment sur les modalités de l’antibioprophylaxie lors de soins bucco-dentaires ; seules des recommandations ont été émises par des sociétés savantes. A partir de ces recommandations et des données actuelles de la littérature, nous nous proposons d’établir des schémas thérapeutiques, pour les patients porteurs de prothèse articulaire ou candidat à la chirurgie, dans différentes situations.

Méthodologie : Etude rétrospective avec classification des infections "a priori" selon le germe isolé dans la prothèse : infection primaire (IP) si isolement de Staphylocoque à coagulase négative (SCN), infection indéterminée si Staphylocoque aureus (Sa) et infection secondaire (IS) si autre pathogène. Etude comparative des infections primaires versus infections secondaire et recherche de facteurs discriminants pour le diagnostic d'infection secondaire parmi les données suivantes : intervalle libre, foyer infectieux à distance, acte invasif dans les 3 mois et hémocultures positives. Résultats : L'analyse a porté sur 141 infections (99 prothèses de hanches et 42 prothèses de genoux) chez 68 hommes et 73 femmes avec un âge moyen de 74 ans. Avec la classification, 34 (24 %) infections sont des infections primaires, 60 (43 %) des infections secondaires et 43 (33 %) des infections indéterminées.
