Book excerpt: De nombreuses avancées concernant l'asepsie et la prise en charge pré, per ou post opératoire ont nettement diminué le risque d'infection du site opératoire (ISO) en Neurochirurgie, et ont amélioré la prise en charge de ces ISO. Cependant ce type d'infection restent un problème de santé publique responsable d'une mortalité, d'une morbidité et d'un coût économique importants ainsi que d'une durée d'hospitalisation plus longue et d'un risque de réhospitalisation plus élevé. Les méthodes de préventions et de prise en charge en sont codifiées. Dans une première partie, nous faisons un état des lieux des ISO en Neurochirurgie, de leur prise en charge préventive et curative. Dans une seconde partie, nous nous interrogeons sur nos résultats en étudiant une cohorte de 19 949 interventions réalisées de 2002 à 2017 au CHU de Limoges. Les facteurs étudiés sont l'âge, le sexe, la classification d'Altemeier, la classification ASA, la durée opératoire, la durée d'hospitalisation pré opératoire, le type de chirurgie, l'opérateur et la saison. Nous mettons en évidence un taux d'ISO moyen de 2,06% sur la période étudiée ce qui est superposable aux données de la littérature. Nous trouvons trois facteurs de risque d'ISO ; l'âge, le sexe et la durée du geste opératoire. Il est intéressant de noter des facteurs de risque différents selon que l'on étudie la chirurgie rachidienne ou cérébrale. Par ailleurs il semble qu'un facteur « identité du chirurgien » soit à approfondir de même que l'impact des données PMSI des patients. La création d'un système d'alerte semble quant à elle, pertinente.

Nos résultats sont comparables à Ia majorité des données déjà connues, mais permettent de mettre en évidence les interventions nécessitant une surveillance spécifique, pour en limiter les risques infectieux.

In order to avoid misunderstanding these abbreviations, it is wise to refer to a reliable dic tionary, such as this one prepared by Heister. The abbreviation ED means, for instance, effective dose to the pharmacologist. However, it might also stand for emetic dose. Radiologists use the same abbreviation for erythema dose, and ED could also mean ethyl dichlorarsine. A com mon meaning of ECU is European currency unit, a meaning that might not be very often in scientific medical publications. ECU, however, also means environmental control unit or European Chiropractic Union. Hopefully, those making inventions and discoveries will make use of Heister's dictionary before creating new abbreviations when preparing manuscripts for scientific publications. It is a very worthwhile goal not to use the same abbreviation for several different terms, especially if it is already widely accepted to mean only one of them. It may be impossible, however, to achieve this goal in different scientific disciplines. Therefore, although it is wise for the abbreviations used in a publication to be defined, it is also very helpful for readers and writers to use a dictionary such as this one. The author deserves our warmest thanks since we know that compiling such a comprehensive dictionary is based upon incredibly hard effort.
