Book excerpt: Position du problème et objectif(s) de l'étude : les traumatismes sévères constituent la première cause de handicap et de mortalité chez le sujet âgé de moins de 35 ans. Les infections de site opératoires (ISO) sont fréquentes dans cette population mais sont pourtant peu étudiées et peuvent grever le pronostic de ces patients. L'objectif de cette étude rétrospective multicentrique était d'étudier l'épidémiologie de ces infections et de mettre en évidence des facteurs de risque d'ISO chez les patients traumatisés sévères. Matériel et méthodes : nous avons mené une étude de cohorte rétrospective multicentrique chez des patients admis dans deux services de réanimation des CHU Nord et Timone à Marseille en 2018 et 2019 et opérés dans les 5 jours suivant l'entrée d'une chirurgie orthopédique ou rachidienne. Nous avons classé les patients en deux groupes : ISO et non ISO selon les critères diagnostiques des Centers for Disease Control (CDC) et avons recherché des facteurs de risque d'ISO liés aux patients, à leurs résultats biologiques et à leur prise en charge initiale réanimatoire, anesthésique ainsi que plus tardive en soins non critiques. Nous avons par ailleurs réalisé une étude des profils bactériologiques et des traitements des différentes ISO ainsi qu'une étude de la survie dans ces deux populations. Une déclaration d'exploitation des données au PADS a été faite (HG5ZSS). L'analyse statistique a été réalisé sous JMP (version 13). Résultats & Discussion : sur la période d'étude (2018-2019), 207 patients ont été hospitalisés dans les deux services de réanimation étudiés avec une prise en charge chirurgicale orthopédique ou rachidienne en urgence. Vingt-trois pour cent (n=47) d'entre eux ont développé une ISO. Les variables les plus significatives dans notre étude étaient la transfusion de CGR, l'hypoxémie le jour de la chirurgie et l'albuminémie élevée à J3 de l'entrée avec des OR respectifs de 1,1 ((1,04--1,17) ; p
